Multivariate COX Analysis Of The Prognosis Effect Factors Of Retroperitoneal Soft Tissue Sarcoma Surgery

Objective To explore postoperative prognosis factors on retroperitonealsoft tissue sarcoma.in order to help to reduce local recurrence rate,improvedisease-free survival and overall survival.Methods To review71cases of retroperitoneal soft-tissue sarcoma, in theAffiliated Cancer Hospital of Guangxi Medical University between Jan.2002and Oct.2013. In the71patients,30underwent complete resection(42.2%),23,combined multiple organ resection (32.4%),9,Palliative resection(12.7%),9,needle biopsies or surgical biopsies (12.7%). All dada were analyzed usingSPSS16.0statistic software,the differences in estimated overall survival,disease-free survival and progress-free survival with Kaplan-Meier methods,Log-rank test and Cox proportional hazards regression model.Results In the patients who underwent en bloc resection(completeresection and combined multiple organ resection),1-year,3-year,5-year localrecurrence rate of retroperitoneal soft tissue sarcoma was26.7%、86.7%,100%%respectively, median follow-up recurrence time was17.5months, overall survival was100%、93.3%、40%, median overall survival time was56months,1-year,3-year disease-free survival was83.4%、33.4%.Univariateanalysis revealed that the prognosis factors were primary or recurrence cases,tumor size, positive or negative surgical margin, differentiated degree,AJCCstage and surgical approaches.The COX regression multivariate analysis showedthat surgical method(P=0.045) and AJCC stage(P=0.05) had significantinfluence on local recurrence rate,and positive or negative surgical margin(P=0.050), surgical approaches(P=0.019), and AJCC stage (P=0.035) were theprognosis risk factors on overall survival.Conclusions1. Early diagnosis of retroperitoneal soft-tissue sarcoma is difficult, en blocresection is difficult, postoperative relapse rate is high.2.En bloc resection is the most effective surgical approaches.3.The prognosis factors were associated greatly with positive ornegative surgical margin, AJCC stage and surgical approaches.
