Part One:The MR diagnosis for breast diseasesPURPOSE The aim of our study is to discuss the MR-DEC features and ADC value of breast benign and malignant for get an involvement of level of diagnosis and differential diagnosis. MATERIALS AND METHODS48women with breast diseases underwent MR-DCE and DWI and being included to study. To analyze the differential of MR findings of breast diseases for find a differential between both, those indexes include form, sign, TIC and apparent diffusion coefficient value. RESULTS [1] All of patients, including28breast cancers(58.3%),12adenoma(25%),4adenosis(8.3%).3fiber of cystic hyperplasia(6.25%),1Pyogenic glaucoma(2.08%).[2] Breast cancer include non-mass18(64.29%) and like-mass10(35.71%), rim enhancement account for16(59.3%) and the ratio of speculation in breast cancer is16(59.3%). Most of adenoma (75%) has an integrated membrane on the T1weighting imaging and most of adenoma appeared uniformity mild-to-moderate enhancement sign on the contrast enhancement imaging. Fiber of cystic hyperplasia is characteristic by single or multiple cysts with distribution along duct. The sign of pyogenic glaucoma is heterogeneous on the contrast enhancement imaging and ring-enhancement scattered being seen, which is deemed to breast ducts.[3] The proportion of Ⅱ TIC in breast cancer is17(60.7%), the III is11(39.3%). The proportion of I TIC in benign is15(88.2%), the II is2(11.8%). As for II TIC, an increasing statistical significance was found in the ratio of II TIC between malignancy and benign(x2=12.547, P<0.05).[4] The mean apparent diffusion coefficient value of gland is1798.8±157.4mm2/s, of benign is1407.5±228.3mm2/s, and of breast cancer is1170.85±114mm2/s, a trend towards statistical significance(t=2.534, p=0.018) was seen among three group. The critical apparent diffusion coefficient value is1467.75mm2/s. CONCLUSIONS The MR features of breast malignant and benign has a lot of different and we can make an accurate diagnosis for breast cancer through analysis MR features synthetically. Part two:the correlations between the MRI features ofbreast cancer with the expression of ER, PR, HER-2and Ki-67PURPOSE The aim of our study was to investigate the correlation between the MR features of breast cancer and prognostic factors including ER, PR, HER-2and Ki-67. MATERIALS AND METHODS We reviewed the magnetic resonance images of28women with histopathologically verified invasive breast carcinomas and summarized each factors of patients. Statistical analyses were performed to explore the associations of these patterns (size, form, rim enhancement, speculated margin and TIC) with common prognostic factors including ER, PR, and Her-2and ki-67. RESOULTS [1] The expression positive ratio of tumors markers include ER19(67.86%)、 PR18(64.29%)、Her-215(53.57%)and Ki-6717(60.71%), respectively.[2] There was no significant relation non-mass-like/mass-like and rim enhancement/non-rim enhancement with prognostic markers.[3] An increasing statistical significance(x2=5.073,p<0.05)was found in positive ratio of Her-2between the size of breast cancer>2cm and size≤2cm, that(x2=6.857, p<0.05)was seen in positive of Her-2between the speculated margin and non-speculated margin.[4] A trend towards statistical significance (X2=7.337, P<0.05)was found for the ADC value differential between Her-2positive and negative. CONCLUSIONS The MR performance of breast cancers (including size, speculated margin, TIC and ADC value) are related to prognostic factors (PR, Her-2) and these patterns may be valuable in the evaluation of breast cancers therapy.
