Objective There are a variety of disease entities of diffuse lung disease (DLD), and the clinical diagnosis for diffuse lung disease often by means of clinical manifestations and imaging findings, but it is insufficient to establish a definitive diagnosis. It is essential to conduct the biopsy technology, including transbronchial lung biopsy and (or) surgical lung biopsy for the definitive diagnosis. The aim of our study was to discuss the diagnostic effect of transbronchial lung biopsy on the diffuse lung disease.Methods19cases with diffuse lung disease in department of respiration of Qilu Hospital of Shandong University since February2012were collected. All the patients have executed transbronchial lung biopsy, including9males and10females. The age of patients was76years old at maximum and36years old at minimum, and the mean age was53years old. All the patients possess the undiagnosed characteristics based on the medical history, the symptoms and signs, X-ray and chest CT, bacteriology, sputum cytology and ordinary bronchoscopy under direct vision. A clinical analysis for the19cases above was conducted.Results The lung tissue was successfully achieved for19cases of patients by transbronchial lung biopsy, and the rate of access was100%.8cases were diagnosed by TBLB in19patients with diffuse lung disease. In the diagnosed patients,2cases were tuberculosis, and5cases were cancer, and the other case was eosinophilic pneumonia. A small number of patient present bloody sputum on the same day or the next day after transbronchial lung biopsy, and the amount of bleeding was less than15ml.Pneumothorax, hypoxemia, arrhythmia, infection, fever and other complications did not arise.Conclusion The transbronchial lung biopsy is an important technique with many specialties such as little trauma, less cost, good repeatability and high security. The diagnostic rate for infection and cancer in diffuse lung disease was higher by transbronchial lung biopsy. The diagnosis for diffuse parenchymal disease usually presents as the non-specific pathology by transbronchial lung biopsy, but there is some value for guidance in clinic. And it could be used as a routine screening tool before surgical lung biopsy.
