Objective: To evaluate the diagnostic significances of pathological studies from lung samples obtained by different lung biopsy modalities in diffuse interstitialLung diseases(ILDs). Methods: Clinical and pathological data were retrospectively analyzed from patients with ILDs of unknown etiology.Transbronchial lung biopsy(TBLB), CT-guided percutaneous needle aspiration biopsy (PCNA), lung biopsy by video-assisted thoracoscopic surgery(VATS )or open lung biopsy had been performed to obtain lung tissues for a pathologic diagnosis. Results: TBLB was performed in 73patients in which 36cases were diagnosed as interstitial pulmonary fibrosisjcases of UIP,8cases of Tuberculosis,2casesof pulmonary alveolar proteinosis(PAP). PCNA was performed in 28patients in which pathologic patterns recognized in 18. Among 28cases underwent VATS and 20cases underwent surgical lung biopsy, except lcase,other cases Conclusions: TBLB should be considered to be the first choices to exclude infectious , malignant diseases and sarcoidosis masquerading as ILDs. Surgical lung biopsy /VATS is often necessary to make a definitive diagnosis and a histologic classification of IIPs.
