Purpose:1.Research the plasma H2S concentration difference between severe preeclampsia, mild preeclampsia, gestational hypertension and normal pregnant women.2.Discuss the influence factors of severe preeclampsia, mild preeclampsia, gestational hypertension and normal pregnant women plasma levels of H2S.3.Explore the relationship between plasma H2S levels and disease severity of preeclampsia.Methods:Select the74cases of single gestational preeclampsia, gestational hypertension pregnant women and74cases of healthy pregnant women who are hospitalized in the department of Obstetrics of the Second Hospital of Shandong University from April2013to December2013for the study. Excluded those who were combined with pathological pregnancy medical and surgical diseases. According to the diagnostic standard of the second edition of the obstetrics and gynecology, we divided the cases into4groups:group SP(29cases), group MP(22cases), group GH(23cases) and group control(74cases). We collect3ml maternal cubital vein blood in two hours after admission(Outpatient normal pregnant women at8to10am), put it in the airtight tube, mark and inspect it as soon as possible, eventually placed in-30℃refrigerator. Use the technology of ELISA to determinate the concentration of plasma H2S. And all of the data was analyzed by SPSS19.0software with methods of Kruskal-Wallis H test, Chi-square test, ANOVA, and spearman correlation analysis. We defined that statistical significance exists when P value less than0.05.Results:1.The median concentration of plasma H2S in SP group, MP group, GH group and control group are0.064(0.002-0.359) pmol/L,0.211(0.016-0.604) pmol/L,0.232(0.016-1.762) pmol/L and0.309(0.004-1.762) pmol/L, the differences among the four groups was significant(P<0.01). Comparison between any two groups of four further, the concentration of plasma H2S in SP group is significantly lower than other groups(P<0.01), the concentration of plasma H2S in control group is higher than other groups(P<0.01).Between the MP group and GH group plasma concentrations of H2S was no significant difference(P>0.05).2.The spearman correlation analysis between the level of plasma H2S and general clinical data (include age, gestational age, parity, SBP, DBP, urinary protein and whether the use of antihypertensive drugs) shows that in SP group:H2S concentration and systolic blood pressure were negatively correlated(r=-0.389, P<0.05), H2S concentration and diastolic blood pressure, urine protein unrelated(.P>0.05); in MP group:H2S concentration and diastolic blood pressure were negatively correlated(r=-0.559, P<0.01), H2S concentration and urine protein unrelated(P>0.05).; in GH group and the control group, the concentration of H2S and other clinical indicators are unrelated(P>0.05).3.According to the severity of disease, the preeclampsia has two types:severe preeclampsia and mild preeclampsia. The spearman correlation analysis between the level of plasma H2S and the severity of preeclampsia shows that the level of plasma H2S and the severity of preeclampsia were negatively correlated(r=-0.382, P0.01). Conclusions:1. The level of plasma H2S in preeclampsia and gestational hypertension is significantly lower than the normal pregnant women of the same age, gestational age and parity. The level of plasma H2S in severe preeclampsia is significantly lower than the mild preeclampsia and gestational hypertension, and the concentration of plasma H2S in severe preeclampsia patient is the lowest. The difference concentration of plasma H2S between mild preeclampsia and gestational hypertension patient is not significant.2.The systolic blood pressure and plasma H2S levels are significantly negatively correlated in severe preeclampsia. Inconsistent with the theory, diastolic blood pressure and urine protein were not related to plasma H2S levels, maybe because of the number of cases are less.And the diastolic blood pressure and plasma H2S levels are significantly negatively correlated in mild preeclampsia.3.The severity of preeclampsia and plasma H2S levels are significantly significantly negatively, and concentration of plasma H2S in severe preeclampsia is lower than in mild preeclampsia patients.Significances:H2S as a new gaseous signal molecule has an important role in blood pressure. In this study, we provide experimental data for plasma H2S levels of hypertensive disorder in pregnancy and normal pregnant women, and discover the severity of preeclampsia and plasma H2S levels are significantly significantly correlated. If H2S indeed play an important role in the pathophysiology of hypertensive disorders in pregnancy, it is used as an indicator for monitoring the occurrence of early prediction of the disease, even as an antihypertensive drug targeting will be possible in the future.
