Objective:To study the risk factors of macrovascular disease in type2diabetes mellitus (T2DM),in order to provide a theoretical basis for preventingthe occurrence and development of T2DM macroangiopathy and to know thecharacteristics of different macrovascular components.Subject:All subjects were T2DM patients hospitalized at the FirstAffiliated Hospital of Guangxi Medical University, division of Endocrinologyand Metabolic Diseases from May2013to January2014, all of them werediagnosised according to WHO (1999year) diagnostic criteria for type2diabetes mellitus and inspected with ECG or Holter,CT or MRI,neck or lowerextremity vascular doppler ultrasound to assess macrovascular complications,including168cases,82cases were male and86female, with an age between34to83years old,average58.05±11.00years and duration between0.08to29years. Exclude non-T2DM patients, T2DM acute complications, stress and thosewho suffered from hypertension, cardiovascular disease, cerebrovascular diseaseand peripheral atherosclerosis before T2DM appeared.Methods:(1) Collect clinical data of all subjects: gender, age, history of present illness, past history, medication history, smoking history, blood pressure(SBP, DBP), height, weight, fasting plasma glucose (FPG),2-hour postprandialglucose (PBG2h), glycated hemoglobin (HbA1c), lipids, homocysteine (HCY),uric acid (UA) and creatinine (Cr)(.2)All subjscts were divided into T2DM withmacrovascular complications and without macrovascular complications groupdepending on the presence or absence of macrovascular complications tocompare its clinical data; then1and≥2kinds of macrovascular complicationsgroup depending on different components, the clinical data of differentcomponents and T2DM without macrovascular complications group werecompared to find out their characteristics;(3)spss13.0was used for dataanalysis. Measurement data were expressed as mean±standard deviation andmedian (min-max), T test and Mann-Whitney u test were used to compare twogroups, ANOVA and Kruskal-Wallis H test and calibration test standard pairwisecomparison were used to compare multiple samples and chi-square test wereused to compare count data. Logistic regression analysis of two categoricalvariables were used for risk factors. P <0.05was considered statisticallysignificant.Results:(1) Age, duration of T2DM, SBP, TC, TG, HCY in diabetes withmacrovascular complications were higher than those without macrovascularcomplications group, the difference was statistically significant (P <0.05),andthere was no statistically significant between FPG, PBG2h, HbA1c in twogroups.(2) Multiple comparisons of different macrovascular components show that age, duration of T2DM, HCY were statistically significant (P <0.05), themore macrovascular components, the higher of age, duration of T2DM andHCY.(3) T2DM patients with and without macrovascular disease wereconsidered as dependent variables, and age, smoking history, duration, BMI,SBP, DBP, FPG, PBG2h, HbA1c, TC, TG, HDL-C, LDL-C, HCY, UA asindependent variables for logistic (enter method) regression analysis,whichshowed that age, SBP, smoking are risk factors for diabetic macrovascularcomplications.Conclusions:1.the duration of T2DM, average age and SBP, TC, TG, HCYin T2DM patients with macrovascular complications is higher than thosewithout macrovascular complications.2. The older, the longer of theduration,the higher of HCY in patients with type2diabetes,the more types ofmacrovascular complication is.3. Age, SBP, smoking are risk factors for T2DMmacrovascular complications.
