Background and Purpose:Breast disease is the most common and multiple diseases today,breast cancer has become one of the threats to women’s lives high tumordisease; Therefore, the more esoteric features of breast vascular structurevariability has become breast surgery to improve early diagnosis andsurgical treatment of critical basic theory. This study was designed breastvascular structures to use dynamic contrast-enhanced magnetic resonanceimaging while using maximum intensity projection (MIP) technology toreproduce images, show clarity arterial anatomy of the breast helps toclearly identify the vascular anatomy of the breast, it is focused on theidentification of new angiogenesis, and assist clinicians in the diagnosisand discriminate of benign and malignant breast tumors.Materials and Methods:1.Study Select our hospital between February2013-January2014using dynamic enhanced MRI scans of patients with breast disease, bychoosing from500patients enrolled, the number of enrolled patients areby surgery, pathology and clinically proven unilateral lesions,500casesof normal breast,350cases of unilateral breast cancer,150cases of unilateral benign breast lesions. Aged30to77years, the average age was49years. This article does not discuss and analyze the impact of differenttypes of breast pathology of blood vessels.2.Inspection methods Using GE HD1.5T superconducting MRIscanner, using an8-channel phased-array surface dedicated breast coil,be3D-MIP reconstruction after enhanced early image subtraction, gradedaccording to vascular reconstruction images shown(Normal breast bloodvessels, benign breast disease and breast cancer vascular vessels).Analysis on the ADW4.3workstation software with3D-MIP,usingimages were enhanced when the most obvious and unenhanced images tosubtraction, the resulting subtraction image maximum intensity projection(MIP) and get bilateral breast reconstructed image.On the high-resolutiondisplay of breast arterial revascularization multi-plane rotation angles ofobservation,on the workstation consists of three senior experiencedradiologist common to all patients2D image reconstruction and3D-MIPimages of blood vessels statistics, analysis and counting,evaluation ofinternal thoracic, external carotid artery,intercostal artery and its branchesdisplay cases, display vessels used for statistical analysis.Results:There are three main normal breast blood supply route,there is theinternal thoracic artery, lateral thoracic artery, intercostal arteries. Bydynamic contrast-enhanced magnetic resonance imaging technique,breast arterial branches display is: Normal breast internal thoracic artery branches can clearly show the display grade Ⅰ was82%(410/500), grade Ⅱ branches display was72%(370/500),lateralthoracic artery branches showed Ⅰ grade was69%(345/500),grade Ⅱ branch showed41%(205/500),intercostal arteries showed54%(270/500); The Breast cancer internal thoracic artery branchesshowed Ⅰ grade was98%(343/350), grade Ⅱ branch showed85%(297/350), Lateral thoracic artery branches showed Ⅰ gradewas80%(280/350),grade Ⅱ branch showed52%(182/350),intercostal arteries showed65%(227/350); Benign breast lesions inthe internal thoracic artery branches showed Ⅰ grade was86%(129/150), grade Ⅱ branch showed52%(78/150), lateral thoracicartery branches showed Ⅰ grade was69%(103/150), grade Ⅱbranch showed42%(69/150), intercostal arteries showed42%(63/150). Through the breast vascular structures by using dynamiccontrast enhanced MR imaging to analysis and statisticalcalculations, we can conclude that: the normal breast internalthoracic artery showed higher than the lateral thoracic artery and theintercostal arteries, internal thoracic artery is the main blood supplyto the breast arteries.Ⅰ, Ⅱgrade internal thoracic artery of breastcancer, Ⅰ, Ⅱgrade lateral thoracic artery showed were higherthan normal breast,theⅠ, Ⅱgrade internal thoracic artery,Ⅰ gradelateral thoracic artery, intercostal arteries showed higher than benign breast disease, surrounded the breast cancer lesions can see theincreased, thickened, broken and aggregation abnormal bloodvessels network.Conclusion:Breast dynamic contrast-enhanced magnetic resonance imaging andmaximum intensity projection (MIP) reconstruction techniques, couldbetter able to display the mammary artery vascular grade Ⅰ and Ⅱpart of the vascular distribution. Breast cancer rich in blood vessels, Ⅰ,Ⅱ grade vessel branches display higher than benign breast lesions.
