objective:To analysis the morphologic features and enhancement kineties of Breast lesion with three-dimensional dynamic contrast and digital subtraetion MRI and to evaluate the aPPlication of MRI indiagnosing breast cancer.Methods:60 Patients were enrolled in this study, including 37 malignantlesions And 28 benign lesions verified by histoPathology.The imaging series include daxial SE T1WI, axial fat-saturated FSE T2WI, sagitalfat-saturated FSE T2WI and axial dynamically enhanced 3D-FSPGR. The image subtraetion and time-signalintensity curve were aehieved on workstation.The diagnostic criteria for benign and malignant tumors were made for morphology, early-Phaseenhaneementrates, type of dynamic enhance ment curve.The morphologic manifestation, early-Phase enhaneement rates, Peak enhancement rates and time-signal intensity curve were evaluation in diagnosing breast diseases.Result:The majority of lesions can not be detected by MRI scanning, while combining of morphological characteristics, the sensitivity is 75%, the sPecifieity is 69.7% and the accuracy is 71.9% for detection of breast cancer. Most types of benign lesions were round or lobulated, Edge Finish clear boundary,and eccentrie Strengthening enhance uniformity;Malignant lesions are more irregular Shap or burr, obscure border, to Strengthen and enhance mental uneven.The difference of the time-significant (p<0.001). diagnostic sensitivity is 91.7%, speeifieity is 87.9% and accuracy is 89.5%, The difference of early Peak time rate increasing between benign and malignant lesions was statistically significant (U-value were 20.50, P<0.001) the former sensitivity is 87.5% and specificity is 87.9%, accuracy is 87.7%; the latter diagnostic sensitivity is75.0% and specificity is 84.8%, accuracy is 80.7%. Combining of dynamic scanning, the sensitivity is 95.8%, the specificity is 87.9% and the accuracy is 91.2% for detection of breast cancer.Concluseons:Time-signal intensity curve, the peak time and the rate of increase have higher sensitivity and specificity in early identification of benign and malignant breast disease. with enhanced characteristics, the most benign and malignant lesions were given diagnosis,which had important value in the diagnosis and differential diagnosis in the benign and malignant lesions.
