Objective:We investigated the impact of clinical characteristics ofpremature ventricular contraction (the PVC burden,theelectrocardiography characteristics of PVC,the site of origin of PVC, theduration of symptoms)on the Left ventricular ejection fraction inpatients without underlying structural heart disease,so that we can makea suitable treatment for these PVC patients to prevent them fromPVC-induced cardiomyopathy.Methods: We retrospectively studied63consecutive patients (meanage37±18years,40female) with no other cause of cardiomyopathywho accepted ablation of PVCs in the2nd Hospital of JilinUniversity from January08,2010to October30,2013.We analyzed theassociation of a reduced LVEF, defined by LVEF≤55%onechocardiography,with features of PVCs obtained fromelectrocardiography,24-hour Holter monitor and electrophysiologystudy.Result:Patients with reduced LVEF (n=10) as compared tonormal LVEF (n=53) had an increased burden of PVCs (30.3±13.6%vs17.7±13.0%, P=0.007);higher prevalence of non-sustainedventricular tachycardia (VT)[8(80%) vs21(40%), P=0.045]; longerPVC duration (154.3ms±22.9ms vs145.6ms±20.8ms, P=0.03);long duration of symptoms(59±20mons vs24±7 mons,P=0.01).There was no significant difference in age,sex, PVCcoupling interval. Patients with fascicular PVCs (n=5) had higher meanLVEF compared to the others (69.2±4.0%vs57.0±10.0%, P=0.0004).There was no association of LVEF with the origin of PVC, i.e. withleft-bundle versus right-bundle branch block morphologies.Conclusion:In addition to the PVC burden, other characteristicslike a longer PVC duration, presence of nonsustained VT and durationof symptoms might be associated with cardiomyopathy.
