Objective:To explore the correlation between HLA-DR4gene and the outcomes of patients with rheumatoid arthritis.Methods:The Han patients with human rheumatoid arthritis were collected from Inner Mongolia region, in which60patients with poor outcomes, rapidly advanced diseases, sustaining high inflammatory markers, positive specific antibodies, less effective basic treatment, significant manifestations outside the joint, and impaired multiple organs were screened out for observation of HLA-DR4RNA expression by using polymerase chain reaction technique with sequence-specific primers. In the case of gene carrying, the outcomes of HLA-DR4-positive patients were evaluated from the comprehensive indicators including clinical manifestations, laboratory indicators, joint ultrasound,X-ray manifestations, and joint functional classification.Results:For the collected60cases of RA patients, the genetic testing results showed that there were52cases of HLA-DR4-positive patients with the gene frequency of86.7%and8cases of HLA-DR4-negative patients. SPSS13.0software was used for statistical analysis of the results. The joint X-ray of HLA-DR4-positive patients showed significant bone destruction, stenosis in the joint clearance, obvious osteoporosis, joint dislocation and fusion, and sustaining high inflammatory markers; the joint ultrasound showed obvious thickening in intra-articular synovial membrane and rich pannus; it was common appeared rheumatoid nodules, rheumatoid lung, peripheral neuropathy, uveitis, and multiple organ damage of heart, kidney and other organs. Compared with the HLA-DR4-negative group, RA patients in the HLA-DR4-positive group had poor outcomes and less effective basic treatment, thus they should be given early and aggressive treatment combined with biological agents.Conclusion:In conclusion, the HLA-DR4positive rate of RA Han patients in Inner Mongolia region was closer to those in other regions, and HLA-DR4was significantly correlated to the outcomes of RA patients.
