Background:Symptoms of subclinical hypothyroidism maybe mild, but epidemiological studies have shown that subclinical hypothyroidism associated withtype2diabetes and a variety of metabolic disease. The relationship between vascular complications in type2diabetes and subclinical hypothyroidism has become a research hotspot to scholars both at home and abroad,but it has not reached a consensus.Purpose:To explore the function of subclinical hypothyroidism in type2diabetesmetabolism. And to find the risk factor of chronic vascular complications intype2diabetes. To provide new prevention direction and theoretical basis for aseries of metabolic disorders in type2diabetes.Method:Two hundred and forty-eight Patients with type2diabetes were selected who stayed in Department of Endocrinology and Metabolism of First Hospital of Jilin University from December in2011to September in2013,31-86years old. They are115males and133female cases, and themean age is58.44. We divide the research objects into two groups according to whether complicated with subclinical hypothyroidism: combinationof SCH group(42) and simple type2diabetes(206). Record general clinical information: gender, age, smoking history,Body Mass Index,bloodpressure,duration of diabetes, and so on. The next morning after8hours’ fasting, draw elbow venous blood and inspection, measuring triglycerides(TG), low density lipoprotein cholesterol(LDL-C), high density lipoprote in cholesterol(HDL-C), fasting Plasma glucose (FPG), thyroid stimulatinghormone(TSH), thyroid hormones free tri-iodothyronine(FT3), free thyroxine(FT4), glycosylated hemoglobin(HbA1c),albumin-to-creatinine ratio(A/C) and so on. Record the occurrence of vascular disease, diabetic nephropathy and diabetic retinopathy. Analysis the diversity of the indexes andits relevance to chronic vascular complication in type2diabetes. Use theSPSS17.0software for statistical analysis.Result:1.The incidence of SCH in type2diabetes is16.49%. Compared to simplediabetes group, the proportion of women in subclinical hypothyroidism(66.67%)is significant higher than the simple diabetes group(59.51%). The difference isstatistically significant.2.Signs and Laboratory Index: HbA1c is lower and Duration of diabetes,Body Mass Index,TG,TC and LDL-C in complicated SCH group is significantly higher than the simple type2diabetes group. The difference is statistically significant(P<0.05). They are no significant differencein age, SBP, DBP, et al.3.Examination result: The incidence of Blood lipid metabolic disorder,metabolic syndrome,Diabetic nephropathy and large vascular disease is higherthan simple diabetes group, and the difference is statisticallysignificant(P<0.05). Diabetic retinopathy, no statistically difference betweenthe two group(P>0.05).4.Related-to relationships: SCH is an independent risk factor for diabetic vascular disease(P<0.05,OR=2.251) and for diabetic nephropathy(P<0.05,OR=2.158), but not for diabetic retinopathy.Conclusion:1.SCH has a high Prevalence in type2diabetes, especially in olderwomen. 2. SCH makes the body’s metabolic disorders worser and increases therisk for metabolic syndrome in type2diabetes.3.SCH makes Patients have a higher indexs in duration diabetes, BMI, TG,TC and LDL-C in type2diabetes. And the glycosylated hemoglobin is lower.4.SCH may be an independent factor for diabetic main vascular diseaseand diabetic nephropathy.
