Objective: To explore the effect of type 2 diabetes with subclinical hypothyroidism on changes in insulin resistance and related factors,and to accumulate experience for clinical diagnosis and treatment.Methods: A total of 113 patients with type 2 diabetes who were hospitalized in our department from October 1,2018 to September 31,2019 were used as the research objects.General data were collected and biochemical indicators such as the five items thyroid functions,blood glucose,and blood lipid were measured.The results of the five items thyroid function were divided into 40 cases of simple type 2 diabetes group(DM group),33 cases of type 2 diabetes with simple subclinical hypothyroidism group(DM-SCH group),and type 2 diabetes with autoimmune thyroid disease(subclinical hypothyroidism)(DM-AITD group)40 cases,the results of insulin resistance index,blood glucose,blood lipids were compared between the three groups.Results: 1.Compared with the DM group,the DM-SCH group and DM-AITD group BMI values were increased,and the differences were statistically significant(P <0.05);the course of diabetes in the DM-SCH group and DM-AITD groups was longer than that of the DM group.The difference was statistically significant(P <0.05).Compared with DM group,there were no significant differences in gender and age between DM-SCH group and DM-AITD group(P> 0.05).2.Compared with DM group,DM-SCH group had higher TC,TG,LDL-C,FPG,OGTT2 h PG,HbA1 c,FINS,TSH,HOMA-IR,and decreased FT3 and FT4.The differences were statistically significant(P < 0.05).Compared with DM group,TC,TG,LDL-C,FPG,OGTT2 hPG,Hb A1 c,FINS,TSH and HOMA-IR were increased in DM-AITD group,while FT3 and FT4 were decreased.The differences were statistically significant(P <0.05).3.In the AITD group,TPOAb was significantly positively correlated with BMI(P <0.05),and had no significant correlation with HbA1 c,FPG,FINS,TC,TG,LDL,TSH,FT3,FT4,and HOMA-IR(P> 0.05).),TGAb has no significant correlation with BMI,HbA1 c,FGP,FINS,TC,TG,LDL,TSH,FT3,FT4,HOMA-IR.(P <0.05).Conclusion: Patients with type 2 diabetes and subclinical hypothyroidism have a higher insulin resistance index,aggravated glucose and lipid disorders,and increased thyroid antibody may increase the risk of obesity.
