Objective To analyze the expression of CD28、CTLA-4、PD-L1、PD-1on T cellsand the levels of Th1、Th2、estrogen in the plasma with the perimenopausal andpostmenopausal women. In attempt to establish the new evaluation system of the T cellimmune function with the the perimenopausal and postmenopausal women.Methods First part The levels of CD28、CTLA-4、PD-L1、PD-1on T cells weredetermined by Flow Cytometry, including30cases perimenopausal women,40casespostmenopausal women and60cases health control. Second part The levels of thelevels of Th1、Th2and estrogen in the plasma were also determined by Flow Cytometry,30cases perimenopausal women,40cases postmenopausal women and60cases healthcontrol.Results First part. There were statistically difference with the expression levelof CD3+、CD4+and CD4+/CD8+in different groups. By comparing between any twogroups can be found that compared with normal group, the relative and absoluteexpression of CD3+, CD4+and CD4+/CD8+in the perimenopausal andpostmenopause group have a certain degree of dropped significantly. There werestatistically difference with the expression level of CD4+CD28+and CD8+CD28+indifferent groups, with the perimenopausal and postmenopause group were lower thannormal group. There were statistically difference with the expression level ofCD4+CD28+/CD4+CTLA-4+and CD8+CD28+/CD8+CTLA-4+, with perimenopausal group was lower than the postmenopause and normal group. There were statisticallydifference with the expression level of CD4+PD-L1+, CD8+PD-L1+and CD4+PD-1+in different groups, with the perimenopausal and postmenopause group were lower thannormal group. There were statistically difference with the relative expression level ofCD8+PD-1+with perimenopausal group was lower than the postmenopause andnormal group. There were statistically difference of the absolute expression level ofCD4+PD-L1+, CD8+PD-L1+and CD4+PD-1+in different groups, with theperimenopausal and postmenopause group were lower than normal group.Second part. Compared to the normal group, the relative and absolute expressionlevels of Th1in the groups of were significantly elevated(F=9.43,P<0.001;F=4.28,P=0.02) with the perimenopausal and postmenopause group were higher than normalgroup.The relative and absolute expression levels of Th2in the groups of weresignificantly elevated(F=24.71,P<0.001;F=22.45,P<0.001) with the perimenopausaland postmenopause group were lower than normal group. The levels of Th1/Th2in thegroups of were significantly elevated(F=98.14,P<0.001) with the perimenopausal andpostmenopause group were higher than normal group. The levels of E2and Pg in thegroups of were significantly elevated(F=46.84,P<0.001;F=15.70,P<0.001) withthe perimenopausal and postmenopause group were lower than normal group. Thelevels of E2/Pg in the groups of were significantly elevated(F=24.00,P<0.001) withpostmenopause group were higher than the perimenopausal and normal group. Thelevels of FSH、LH and FSH/LH, in the groups of were significantly elevated(F=131.71,P<0.001;F=73.71,P<0.001;F=73.44,P<0.001)with the perimenopausaland postmenopause group were higher than normal group.There was significantly positive correlation between the relative and absoluteexpression level of Th1with the E2(r=0.375,P<0.001;r=0.349,P<0.001). There wassignificantly positive correlation between the relative and absolute expression level ofTh2with the E2(r=0.867,P<0.001;r=0.747,P<0.001).. The level of Th1/Th2hadsignificantly negative correlation with the age(r=-0.497,P<0.001) There wassignificantly positive correlation between the relative and absolute expression level of Th1with the age (r=0.346,P<0.001;r=0.218,P=0.013). There was significantlynegative correlation between the relative and absolute expression level of Th2with theage (r=-0.429,P<0.001;r=-0.447,P<0.001).. The level of Th1/Th2had significantlypositive correlation with the age(r=0.811,P<0.001).There was significantly positive correlation between the relative and absoluteexpression level of CD4+PD-1+with the E2(r=0.571,P<0.001;r=0.523,P<0.001);There was significantly positive correlation between the relative and absoluteexpression level of CD8+PD-1+with the E2(r=0.625,P<0.001;r=0.524,P<0.001)。There was significantly negative correlation between the level of CD4+CD28+/CD4+CTLA-4+with the Th1/Th2. And the level of CD8+CD28+/CD8+CTLA-4+also had a significantly negative correlation with Th1/Th2(r=-0.306, P<0.001;r=-0.357, P<0.001). There was significantly positive correlationbetween the relative and absolute expression level of CD3+with the E2(r=0.341,P<0.001;r=0.227,P=0.009). There was significantly positive correlation between therelative and absolute expression level of CD4+with the E2(r=0.302,P<0.001;r=0.239,P=0.006). The relative and absolute expression level of CD3+had significantlynegative correlations with age(r=-0.516,P<0.001;r=-0.387,P<0.001);The relativeand absolute expression level of CD3+had significantly negative correlations with age(r=-0.520,P<0.001;r=-0.456,P<0.001)。Conclusion1. There were disorders of T lymphocyte subsets with the perimenopausal andpostmenopausal women. The relative and absolute value of CD3+、CD4+、CD8+T cellswas down, So it was implied that to monitor the both expression of cell surface canobjectively reflect the women’s immune function.2. The expression levels of CD28、CTLA-4、PD-L1、PD-1on T cells showeddifferent degree of decline.3. The results showed cytokines in plasma dominant Th1-type cells exist in a stateof the perimenopausal and postmenopausal women. The expression of E2and Pgdecreased while the expression of LH and FSH rised. 4. Some cytokines in plasma and costimulatory molecules on T cells may be usedas a new evaluation system of the T cell immune function with the perimenopausal andpostmenopausal women.
