Objective:To Comparative and analysis the effectiveness after treatment on extension distal radius fractures with three external fixation. To evaluation of the clinical results of plaster and small-splint fixation in the extension distal radius fractures treatment.Methods:90cases extension type distal radius fractures were selected. and randomly divided into three groups (group A、B and C). And the30patients in group A were treated with traditional manipulative reduction combined with small-splint fixation, while maintaining the mild wrist palmar flexion, ulnar deviation. Group B were treated with traditional manipulative reduction combined with plaster fixation, while maintaining the mild wrist palmar flexion,ulnar deviation. The others (in group C) were treated with traditional manipulative reduction combined with small-splint and plaster fixation, while maintaining the mild wrist palmar flexion, ulnar deviation. Then the X-ray films(the wrist anteroposterior and lateral film) were taken in the time before treatment and the first day, first week and month, the third month after the treatment, measured three groups of volar tilt angle, ulnar inclination angle and Radial length, And appropriate adjusted the degree of splints, plaster tightness and fracture location based on X-ray film review the situation and fixed case.meanwhile guide the patients functional exercises. The small-splint, plaster were removed after fractures concrescence and reach the clinical efficiency. We evaluated wrist joints function by Gartland-Werley scores and assessed the clinical efficacy synthetically.All the date were analysed by SPSS17.0statistical software.Results:All patients were followed up for3-6months.Fractures redisplacement were15cases(group A were5cases,group B were10cases);Radial shortening were34cases(group A were15cases,group B were11cases,group C were8cases).Experimental patients were achieved bone healing,Fracture healing time:group A was (5.76±1.33) weeks,group B was (6.50±1.61) weeks,group C was (6.10±1.43) weeks.According to X-ray and Gartland-Werley scores:group A:9cases were graded as excellent,15cases as good and6cases as poor, good rate was80.00%; group B:8cases were graded as excellent,13cases as good and9cases as poor, good rate was70.00%;and group C:18cases were graded as excellent,10cases as good and2cases as poor, good rate was93.33%.Representative cases were founded in Appendix six. Conclusion:The method that traditional manipulative reduction combined with splint plus plaster fixation of extension distal radius fracture is stability and effectinely, It can effectively maintain a relatively high degree of radial, palm inclination, ulnar deviation,and remains the replacement effect until fracture healing. It also can reduce the incidence rate of fracture displacement again.
