Objectives:To terat endometrial polyps in three different operationapproaches of hysteroscopy and evaluate the clinical outcome.Methods:120cases of who diagnosed as endometrial polypby hysteroscopy or transvaginal sonography were systematically reviewed. All120cases were divided into3groups by age and bearing reguirement. Ingroup A,28patients underwent transcervical resection of endometrial polyps,while44patients in groups B underwent transcervical resection of endometrialpolyps and part of endometrium.Group C includesd48patients whounderwent transcervical resection of endometrium.Patients were followed upfor one year to evaluate the outcome of surgery.Result: All120operations were finished. all samples were diagnosed ofendometrial polyp by pathology examination. Average of menstrual quantitywere90.04±23.60ml and42.69±22.26ml for preoperative and postoperative,respectively. The difference was statistically significant with a P value lessthan0.05. Average of menstrual period were7.92±3.05days and4.03±2.16days for preoperative and postoperative, respectively. The difference wasstatistically significant with a P value less than0.05. Average of operation timewere9.54±2.51minutes,15.57±3.93minutes and21.46±5.12minutes forgroup A,group B and group C, respectively. The difference was statisticallysignificant with a P value less than0.05. Average of operation blood loss were8.71±2.34ml,10.93±2.63ml,17.77±6.12ml for group A,group B and group C,respectively. The difference was statistically significant with a P value lessthan0.05.10cases of patients with infertility,7of them wereintrauterine pregnancy within1year after operation, with a intrauterinepregnancy rate of70%. Preoperative symptoms of group A and group B andgroup C there were14cases,37cases,35cases. Postoperative symptom improvement case of group A, B and C were12cases,36cases and35cases.Postoperative symptom improvement rate of group A, B and C were85.71%,97.70%and100%, respectively, showing no significance between the3groups.Postoperative recurrence of group A, B and C were10.71%,2.33%and0, respectively, showing statistically significance between the3groups.Conclusion: As a less invasive procedure, hysteroscopic surgery is thebest treatment for endometrial polyp, with fewer in hospital days and lowerrecurrence rate. There is no significant difference of clinical outcome betweenthree approaches of endometrial polyp treatment, but a higher recurrence isobserved in transcervical resection of endometrial polyps. To obtain betteroutcome, different operation approaches should be applied according topatients’ ages and bearing reguirement. Hysteroscopy should be considered forpatients with infertility to get higher intrauterine pregnancy rate.
