Chronic cor pulmonale is a severe obstructive lung disease,which is mainlycomposed of right heart damage,with the whole heart and systemic diseases,accounting for about18%-37%of all kinds of organic heart disease,which hasrisen from2to the top of the organic heart disease in many areas such as thenortheast and southwest region of chronic cor pulmonale in recent years. Thedisease often occurs in the elderly, and because of their low body reactivity andcourse, of their history of repeated infection and drug use, of their complexpathogen and multiple drug resistance, of their troublesome treatment, it is theimportant reason for damage to the the elderly health in our country. If we canfind an early control infection to improve ventilation and correct hypoxia,chronic cor pulmonale can get to a certain extent, which can effectively reducecomplications and the viscera damage, so the early diagnosis of chronic corpulmonale is particularly important if you can find an early infection which caneffectively detect patients who suffered heart disease from lung disorders, aswell as the monitoring and treatment effect of resistance markers, it willundoubtedly greatly reduce lung case fatality rate.Objective:The purpose of this article is to investigate the levels of b-type natriureticpeptide and allergic C-reactive protein in patients with acute aggravating periodof chronic cor pulmonale,and the relationship between b-type natriuretic peptideand allergic C-reactive protein.Methods:Select70outpatients with the diagnosis for the chronic cor pulmonale with acute aggravating period in the First Clinical Hospital of Jilin Universitybetween February2012and November2012, with the male34cases and female36cases, their average age was67.67±11.67, their basic disease was chronicbronchitis and emphysema. The diagnostic criteria was based on the2007standard. The patients were divided into chronic cor pulmonary lung cardiacfunction in patients with compensatory group A and group B patients withpulmonary cardiac decompensation period. All patients admitted to hospital hadnot been treated with diuretics asoactie drugs boost1week before.In ourhospital30cases of healthy volunteers were chosen as control group C, with theexception of high blood pressure and severe liver kidney brain diseases such ascoronary heart disease,among them,15cases were male, female15cases, theiraverage age was62.73±11.68.Measure the serum b-type natriuretic peptide andhypersensitive c-reactive protein level of the three groups of patients before thehospital determination,after treatment of remission measure the levels of groupA and B again.Compare the changes of serum BNP and hypersensitiveC-reactive protein to see whether there was statistics difference among the threegroups.Results:(1) Compared with the control group,the serum BNP and hs-CRP levels inchronic cor pulmonale patients with acute aggravating period A and B increasedsignificantly before treatment,the difference was statistically significant(P<0.01), and the levels of serum BNP and hs-CRP in patients with acuteaggravating period of decompensated group B was elevated more prominentlythan compensatory group A,the difference was statistically significant (P<0.01).(2) The changes of serum BNP and hs-CRP levels of he two groups ofchronic cor pulmonale decompensation and compensatory phase after treatmentdecreased more significantly than patients who were not treated,theircomparison of BNP and hs-CRP changes between two groups, the difference was statistically significant (P<0.05).(3) There was a positive correlation between the hs-CRP and BNP(r=0.853, P<0.05), the regression equation was y (BNP)=43.273x (CRP)-325.7.Conclusion:(1) BNP is associated with patients with cardiac insufficiency, and it can beused as an early monitoring sensitive indicator of right heart function.(2) The levels of hs-CRP changed with cardiac damage, and it may indicatethe right degree of cardiac insufficiency.(3) The levels of b-type natriuretic peptide and c-reactive protein wereassociated with the patient’s illness severity, and there was a positive correlationbetween them before the patients with acute aggravating period of chronic corpulmonale were treated, the joint detection contribute to the severity of theclinical, analysis of right cardiac insufficiency condition assessment,and toevaluate the effect of the treatment and prognosis.
