| Objective:This study is to assess the efficacy of remedies promoting blood circulation and removing blood stasis (PBC&RBC) for Primary carcinoma of the liver(PLC) based on reviewing the reports of clinical studys,Then to provide the objective,confident result to guide the clinical practice.And the18cases of primary liver cancer patients as the research object, analysis patients’general condition, symptoms,kps, syndrome differentiation and use of sister herbs in Chinese crude drug medicine.The thesis mainly includes the following two parts:The first part is about Meta-analysis of Primary carcinoma of the liver treated with TCM of PBC&RBC.Method:All the literatures of clinical studies that used PBC&RBC as the main intervention to treat PLC were retrived and collected of Meta-analysis was performded by using the statistical soft ware RevMan4.2.10on the basis of strict quality evalution. Sensitivity analysis was carried out between w&rious inclusion criteria. Funnel-plot was used to identify the publication bias.Resultes:Twenty five literatures were selected according with criteria,all papers were randomized controlled trials (RCTs). The Meta-analysis results showed that PLC treated with TCM of PBC&RBC on the overall efficiency, survival rate, quality of life improvement rate, quality of life KPS score, AFP, the role of white blood cell count, Metastasis rate in a year was significantly better than the western medicine treatment alone,the difference was statistically significance (P<0.05) Analysis shows that the results of stability and true and reliable except Metastasis rate in a year. Conclusion:PLC treated with TCM of PBC&RBC can improve clinical symptoms, prolong survival and improve quality of life and a better efficacy and lower side effects than western medicine treatment alone. But,the clinical control study reported in the literature has a lot of problems,the clinical trials less normative etc. make findings of the study is difficult to be recognized.The second part is the retrospective analyzing of the clinical cases about PLC.Method:All18cases about PLC according with criteria were involved in the study from Hunan Tumour Hospital for treatment from Dec.12012to Dec.312013,and took TCM herbs.Resultes:18cases of PLC patients mostly got exhausted (15cases), pain (11cases), poor appetite(13cases) abdominal distension discomfort(13cases), jaundice(4cases) for common symptoms. After taking traditional Chinese herbs,the improvement rate of common symptoms:pain at36.36%,exhausted at60.00%,poor appetite at53.85%,abdominal distension discomfort at69.23%,jaundice at75%.Frequency of18cases of PLC patients Traditional Chinese Medicine does prescription are:huangqi is54,baizhu is54,yiyiren is51,chaomaiya is49,ji’neijin is49,tubiechong is48,cubiejia is48,baihuasheshecao is48,yujin is47,banzhilian is46,dangshen is46,chepi is45,xiangfu is45,shanyao ia45,ezhu is43,sumu is43,dahuang is42,sanqi is42,paojiang is42,yanhusuo is42.After taking traditional Chinese herbs,18patients improvement rate of KPS are50%.The patients generally got physical situation increased, increased appetite, local pain mitigated, relieve symptoms commonly. Conclusion:After taking Chinese herbs, the common symptoms of18cases of PHC were generally improved, and also had differences between different symptom scores,symptom of score of1and2are obviously improved.But in cancer pain isn’t improved,espacilly the pain score attained3need to take the painkiller.The improvement rate of patients whose KPS were more than60grade is better,but some of the patients had poor effects because the illness is heavy and course of treatment is late. |
