Experimental Research And Clinical Observation Of Heat-clearing And Toxin-removing Therapy On Treating Gastric Ulcer

ObjectiveThis topic is for the purpose of seeking the traditional Chinese Medicine to heat-clearing and toxin therapy to treat gastric ulcer’s curative effect. Through we stduy and contract in aspects of pathology,serology,immumnohistochemical and so on,afer gastic ulcer rat treatment,as well as in the gastric ulcer patient’s curative effect observation,searching for the function mechanism and clinical application value of Huanglian Jiedu Decoction on promotion of recovery of peptic ulcer.Methods1.Animal experimentationSD rat90,weight(180±20)g,half male and half female, were divided randmoly into6groups.They are named the blank group.the modle group,the postive group,the high dose group of Huanglian Jiedu Decoction,the middle dose group of Huanglian Jiedu Decoction, the low dose group of Huanglian Jiedu Decoction,each group of15rats.Except the blank group, rats in the other group were gavaged the suspension of cayenne peper and ethanol to establish the heat syndrom.Then submucous injection100%glacial acetic acid to estable the gastic ulcer model.Afert achieve molding,the blank group and the modle group were gavaged0.9%NS, the other groups were gavaged homologous medicinal. Afer2weeks, take the femoral arterial blood to detect the indexes of EGF、TGF-a.Take out of the stomach,scissoring the stomach along grester curvture.Measueing the ulcer area by vernier caliper.Taking the ulcer margin cut into slices,then watch the healing of the ulcer by the microscope. Detecing the indexes of NF-κB、iNOS by immumnohistochemistry.All datas were analysised by SPSS17.0.2.Clinical observationOut-Patients from December2011to January2013which are conformed to the selected standard,were treated with the modified Huanglian Jiedu Decoction. The main observation include clincal symptoms and adverse reaction.Results1.Pathology:Compared with model group, all dose groups can reduce the rat gastric ulcer area(P<0.05or P<0.01), there were obvious differences in the high dose group of Huanglian Jiedu Decoction and the ranitidine group(P<0.05); Compared with model group, Huanglian Jiedu Decoction groups can reduce the rat gastric tissue pathological score、infammation score and fibrosis score(P<0.05or P<0.01),increase granulation score(P<0.05orP<0.01).2. The indexes of EGF%TGF-α:Compared with model group, the Huanglian Jiedu Decoction groups can enhance the indexes of EGF、TGF-α(P<0.05or P<0.01). the postive group ranitidine can also reduce the indexes of EGF、TGF-α (P<0.05).3. The indexes of NF-κB、iNOS protein in rat’s gastric tissue:Compared with model group, the Huanglian Jiedu Decoction groups can reduce the indexes of NF-κB、iNOS (P<0.05or P<0.01), the postive group ranitidine can also reduce the indexes of NF-κB、iNOS protein(P<0.05).4.Clinical observation:the modified Huanglian Jiedu Decotion can obviously reduce the score of clinical syndrome,TCM Syndrome effective rate is90.625%.During the treatment there were no adverse reaction.ConclusionOn the one hand,Huanglian Jiedu Decotion can enhance the rat’s indexes of EGF、TGF-α,thus to promote regeneration and repair of mucous eptithelium;on the other hand, Huanglian Jiedu Decotion can reduce the express of NF-κB、iNOS protein in rat’s gastric tissue,thus to lighten inflammatory response, conduce to ulcer’s healing. On clinical observation,it can improve clinical symptoms and no adverse reaction.
