Background:Chronic apical periodontitis is mainly an inflammatory decease of infective bacterial etiology, which is a highly prevalent disease. Chronic apical periodontitis is associated with significant up-regulation of multiple pro-inflammatory cytokines including Interlekin-1β (IL-1β) and tumor necrosis factor alpha (TNF-α). Calcium hydroxide is a common root canal disinfection drugs, the study pointed out that the periapical effect of calcium hydroxide on Enterococcus faecalis and other factors caused poor. Allicin derived from natural plants, a good anti-bacterial and anti-inflammatoryObjectivesThe aim of this study was to determine the expression changes of TNF-α and IL-1β in root canal of chronic periapical periodontitis patients before and after short applications of sealer (calcium hydroxide or garlic), to compare the efficacy of anti-bacterial action of these two agents.Methods21teeth involved with chronic periapical periodontitis collection in the endodontic Department of Xiangya Hospital of Central South University. They were randomly divided into three groups sealer material in the root canal1-Experimental group (garlic group) sealed by garlic in the root canal orifices,2-Experimental group (calcium hydroxide) sealed by calcium hydroxide in the root canal orifices,3-control group (normal saline) sealed by saline in the root canal orifices Collect root canal exudate sampling before and after sealing medicine by Two sterile paper point was inserted into the root canal place for30second, after than Paper points were immersed into sterile Ependorf vials and stored at-70℃until further analysis.detected of TNF-α and IL-1β expression levels in root-canal exudates and by enzyme-linked immunosorbent assay. Statistics done by Spss16.0continues variables expressed as mean and standard deviation. Categorical variables expressed as frequencies, univariate analysis using different variables. IL-1β, TNF-α levels of pre/post treatment in each patient were compare the data between two groups t-test were used. For more than2groups ANOVA test was used. Results Concentration IL-1β before and after drugs sealing for three groupsComparison between the expressional levels of IL-1β before and after treatment in the three groups. The differences in the Garlic and Calcium hydroxide was interesting nearly equal, reflecting the high value for using Garlic as root canal sealer when compared with the Calcium hydroxide which considered the standard root canal sealer.Concentration TNF-a before and after drugs sealing in three groups The comparison between the expressional levels of TNF-a before and after treatment in the three groups. The differences in the Garlic and Calcium hydroxide was interesting nearly equal, reflecting the high value for using Garlic as root canal sealer when compared with the Calcium hydroxide which considered the standard root canal sealer.Conclusion1-In chronic apical periodontitis, the TNF-α and IL-1β levels will be increased in root canal exudates of the teeth2-Garlic and calcium hydroxide can decrease the TNF-α and IL-1β levels in the root canal exudates effectively3-Both garlic and Calcium Hydroxide play the same role in reducing the levels of TNF-α and IL-1β...
