objective: As the aggravation of population aging, femoral neck fracture casesincreased year by year for the aged people. Due to the particularity of the elderlypatients body. What kind of treatment method is more suitable for elderly patients in thisspecial group, the Article selected2surgical approach of femoral neck fracture forelders. Comparison of hollow compression screws fixation, hip replacement as theTreatment method in elderly patients with femoral neck fracture, which provide basisfor surgeon to find a suitable treatment.Method: From January2010to December2011,113cases of elderly patients withfemoral neck fracture were treated. There are55cases by using Limits the heartcompression screw fixation (Hollow compression screws group),26males and29females, aged74.71±6.22years old. Fracture Classifications in Clinical Practice,Garden I+II in45cases, Garden III+IV in10cases. During the time of intervalfrom injury to the operation time,12h to12d, average value with4.2d.58cases ofRoutine hip replacement (Hip arthroplasty group).23males and45females; aged73.73±8.16years old. Fracture Classifications in Clinical Practice, Garden I+II in9cases, Garden III+IV in49cases. During the time of interval from injury to theoperation time1-14d. with an average4.4d. Make a comparison for Operation Time,intraoperatve blood soss, Blood transfusion rate, length of stay, postoperativecomplications, mortality rate, Reoperation rates and the grade of Hip joint functionrespectively. that of hip arthroplasty group Result: Operating Time of Hollow compression screws group, Intraoperative blood,hospital days loss is superior to that of hip arthroplasty group, statistical difference (P <0.05). Patients were followed-up for postoperation, follow-up time is12month to24month,7month annual year in average. Occurrence rate of early complications forHollow compression screws group is14.54%(8/55. Occurrence rate of earlycomplications for Hip arthroplasty group is5.17%(3/58).Late complication rate are.7/55(12.72%)2/58(3.45%)respectively. After1-2years of postoperation, There are3cases of patients with hip replacement surgery for Hollow compression screws group.Reoperation rates is5.45%.There is no reoperation case for Hip arthroplasty group.After comparing for2methods, for early period and late period of postoperation,Postoperative Complications and reoperation rate for Hollow compression screws groupis higher than Hip arthroplasty group. Differences have statistical significance(P<0.05).After1-2years of postoperation, there is no statistical significance in comparison ofmortality rate for Hollow compression screws group and Hip replacement0.05).Gradehip joint function for patients are in live by Harris Hip Score. The fineness rate ofHollow compression screws group is64.70%(33/51)and Hip replacement group is81.48%(44/54).Differences have statistical significance between two groups (P<0.05).Conclusion: For elderly patients with femoral neck fracture, to choose hip arthroplastygroup can reduce the postoperative complications and postoperative overhaul ofprobability significantly. After1-2years of postoperation, the fitness rate of Hip jointfunction score is higher than the hollow compression screw internal fixation.Comparingwith Hollow compression screws fixation, although hip arthroplasty requires muchmore Medical expenses Hollow compression screws fixation, can make the patient leavethe sickbed earlier, improve the quality od life, and avoid the necrosis of femoral headand the risk of fracture nonunion.So, for elderly patients with femoral neck fracture whoexpect a longer lifetime and much more requirements for activity of daily living,artificial hip replacement is the better choice.
