Objective: To study and analyze the characteristics of different types of benignparoxysmal positional vertigo (BPPV) in videonystagmograph (VNG), deepen theunderstanding of the clinical characteristics of BPPV and explore the possibleetiology and pathogenesis.Methods:126patients diagnosed with BPPV of ENT clinic in the SecondAffiliated Hospital of Dalian Medical University Department from October2011–December2012were examined by nystagmus view and then we compared and analyzedthe results of saccadic BPPV in patients with nystagmus view saccade test, stabletracking test, optokinetic nystagmus test (OKN), gaze test and caloric test.Results: the optokinetic system check, including gaze saccades optokinetic test andsmooth tracking examination, the study group and the control group had no statisticalsignificance. Spontaneous nystagmus test, the study group and the control group had nostatistical significance. Caloric test, study group: within the126patients with BPPV,67patients with abnormal CP (53.2%),20patients with abnormal DP; control group:100healthy people, only9cases appeared abnormal CP. The study group and the controlgroup had statistical significance. Within67cases of CP abnormal,22male cases,45female cases, there is no statistical significance. In the abnormality of the CP age group,only40~49years and50~59years old groupχ2=8.248, P=0.004, P<0.05, there wasstatistical significance. The rest of the two groups were compared by P values and itwas more than0.05, having no statistical significance. Within the67cases of CPabnormal,47cases (71%) of abnormal CP and BPPV prevalence were the same side,20cases were different side (29%), the same side is higher than the opposite side. Therewere21cases (52.5%) of the lateral semicircular canal BPPV and26cases of(30.9%)posterior semicircular canal BPPV, lateral semicircular canal BPPV in patientswith abnormal CP was much higher than that of posterior semicircular canal BPPV. Conclusion: Vestibular bithermal caloric testing plays an important role in thediagnosis of patients with BPPV.The results of dual temperature test were not related tosex, but to the age. BPPV start high from ages50to59years old, abnormality rate ofdual temperature test is also increased significantly from this age.Abnormality rate ofdual temperature test of lateral semicircular canal BPPV patients was significantlyhigher than that of the posterior semicircular canal BPPV patients, BPPV patients withparesis or ipsilateral also may not be consistent, explained the effect of temperature onthe semicircular canal is not only because the fluttering of otolith, but also becausethe vestibular peripheral disease.
