Objective Through three-dimensional color power angiography on ovarian masses,observed the blood flow distribution characteristics and determine thevascular parameters, further explained the3D-CPA three-dimensional,intuitively reflect the ovarian blood supply of the tumor, the preoperativedifferential diagnosis of benign and malignant ovarian has importantdiagnostic value.Methods30cases of ovarian tumor in27patients by two-dimensional ultrasounddiagnosis of ovarian tumors in preoperative three-dimensional color powerangiography examination, observation of tumor vascular distributioncharacteristics of vascular grade of them, and through the built-in VOCALsoftware three-dimensional ultrasound to calculate its vascularization index(VI), flow index (FI), angiogenesis blood flow index (VFI) three vascularparameters, and the results were compared with pathological, evaluationbetween the benign and malignant, ovary tumor vascular parameters haveno difference.Results (1) Sensitivity of three-dimensional color power angio ultrasound in the differentialdiagnosis of ovarian malignant tumor was91.6%, the specificity was88.9%, thepositive predictive value of84.6%, negative predictive value of94.1%, the coincidencerate is90%.(2) Benign ovarian tumor groups0, I level flow primarily,malignant ovariantumor groups II, III level flow primarily. Blood flow in benign ovariantumor group the detection rate of83.3%(14/18), the detection of malignanttumor group of blood flow rate of100%(12/12). There are significantdifferences between malignant ovarian tumor vascular morphology and vasculargrading and benign ovarian tumor P<0.05, and class III vessels as the sensitivity in the diagnosis of malignant ovarian tumor was58.3%, the specificity was88.9%, thepositive predictive value of77.8%, negative predictive value of76.1%, compliance ratewas76.7%.(3) VI, FI, VFI value between benign group and the malignant group,the difference wasstatistically significant (P<0.05), with95%confidence interval of the upper limit oflimits of diagnosis of malignant ovarian tumors, namely VI>9.157(%), FI>36.753(0.1),VFI>3.699(0.1) for the diagnosis of malignant tumors, the sensitivity was66.7%,specificity94.4%, the positive predictive value of88.9%, negative predictive value of81%, compliance rate was83%.Conclusions Three-dimensional color power angiography imaging for preoperativedifferential diagnosis of benign and malignant ovarian tumors providesan important basis for the diagnosis, provides the basis for earlydiagnosis of ovarian malignant tumor, because it can reflect stereoly,intuitively the bloody supplying of ovarian tumor.3D-CPA can guide theposition and coverage of surgery,which has guiding significance to theclinical diagnosis and treatment and operation plan.
