Part I Influence factors of plasma homocysteine level in women with polycystic ovarian syndrome[OBJECTIVE]Through testing folate levels, thyroid function, Homeostasis Model Assessment of Insulin Resistance and homocysteine levels, we aim to compare the metabolic indexes between PCOS women and the control group, evaluate the influence factors of plasma homocysteine level in this woman, and provide relevant recommendation for comprehensive treatment of PCOS patients.[METHODS]1. MaterialsClinical data were collected from80PCOS patients diagnosed in Reproductive Center of Navy General Hospital from June2012to June2013. And107childbearing aged women with normal menstrual cycle and ovary function were collected during the same period.2. Diagnostic Criteria and Exclusion CriteriaDiagnostic Criteria and Exclusion Criteria of PCOS was based on2003Rotterdam criteria. All participants haven’t taken folate, vitamin and hormone agents in the past three months.3. Choice of relevant factorsEleven independent variables were selected according to the report of reference literature and the experience in our center.Age, Body Mass Index (BMI), Systolic Blood Pressure (SBP), Diastolic Blood Pressure (DBP), Pulse Rate (PR), Folate, Free Triiodothyronine (FT3), Free Thyroxine (FT4), Thyroid Stimulating Hormone (TSH), Homeostasis Model Assessment of Insulin Resistance (HOMA-IR), Homocysteine (HCY).4. Statistical AnalysisData were statistically analyzed with the SPSS13.0software package using Shapiro-Wilk W test for the normality of distribution for continuous variables and data were expressed as mean±standard deviation. Means were compared between groups by Independent-Sample t-test. Multiple stepwise regression analysis was performed to assess the significance of the correlations and the independent contributors to the variance of HCY levels. Take a=0.05as the inspection level, P values<0.05were considered statistically significant.[RESULTS]1. Comparison of general characteristics between the PCOS group and the control group (x±s)From Independent-Sample t-test, there was a significant statistical difference between PCOS group (n=80) and control group (n=107) for BMI (P<0.001), SBP (P<0.05), DBP (P<0.001) and PR (P<0.05). That is, age in PCOS group was significant less than the control group, and BMI, SBP, DBP as well as PR were significant higher than the control group.2. Comparison of biochemical indices between the PCOS group and the control group (x±s) From Independent-Sample t-test, there was a significant statistical difference between PCOS group (n=80) and control group (n=107) for folate, HCY, FT3, TSH and HOMA-IR (P<0.05) except FT4. Folate in PCOS group was significant less than the control group, and HCY, FT3, TSH as well as HOMA-IR were significant higher than the control group.3. Comparison of biological indices of in PCOS patients when divided by TSH (x±s)When divide the PCOS subjects into two group by TSH levels of2.5,3.0,3.5or4.0IU/L respectively, there were no significant difference in HCY levels between groups. When divided TSH levels by3.5IU/L, HCY in TSH>=3.5group (n=22) was higher than in TSH<3.5group (n=58) though no statistically significance (P=0.077). When divided TSH levels by4.0IU/L, there were no significant difference for HCY and folate levels between TSH>=4.0group and TSH<4.0group (the results were not listed because there were only13patients in TSH>=4.0group).4.
