Study On Special Dietary Guidance Effects Recurrence Rate Of Urinary Stones

Objective: To study effects of special dietary guidance in prevention urinarystones; To study recurrence rates of different gender, age, education, occupation,marital status of patients following different dietary instruction for12months and24months, as to provide theoretical basis for clinical personalized diet therapy.Methods: A total of380people with urinary stones treated at department ofurology of China-Japan union hospital between January2010and January2010wereselected, consist of180males and100females,72patients with renal stones,257withureteral stones and51with both. Divide the patients into2groups matched by gender,age, culture level, occupation, marital status. The experimental group followed thespecial dietary guidance, base on the analysis of the calculi ingredients, and thecontrol group take the normal dietary. The follow-up period of patients was24months,the Urinary tract ultrasound, CT, routine urine check. All were taken to decide if thecalculi recurred or not. Measurement data using mean±SD, measurement data iscompared between groups using t test, enumeration data using constituent ratio,measurement data is compared between groups using χ2test.Results:1.With the follow-up period of12and24month, the recurrence rate ofthe experimental group was6.8%and10.1%, compared with the control group was22.1%and24.7%, the difference is significant.2. With the follow-up period of12and24month, the recurrence rate of males in the experimental group was7.9%,9.2%,and the females was5.8%,12.5%, while the control group was20.8%,20.5%, and26.2%,36.6%. There is statistically different for males.3. With the follow-up period of12and24month, the recurrence rate of the experimental and control group is23.1%,7.7%and22.1%,22.2%at the age of20s;6.8%,13.8%and19.3%,21.6%at30s;4.7%,8.2%and27.3%;28.6%at40s;4.2%,4.5%and19.2%,23.1%at50s; and6.3%,14.3%and20.0%,28.6%at60s.4. With the follow-up period of12and24 month, the recurrence rate of junior high school and below sub-group of experimentwas4.2%,12.5%, control was16.7%,13.6%; high school or technical secondaryschool sub-group of experiment was10.8%,14.3%, control was20.3%,27.4%;college or undergraduate sub-group of experiment was4.7%,5.2%, control was21.1%,21.8%; graduate and above sub-group of experiment was4.3%,8.7%, controlwas36.4%,36.8%.5. With the follow-up period of12and24month, the recurrencerate of civil servants sub-group of experiment was2.7%,10.8%, control was20.6%,21.9%; famers sub-group of experiment was0%,0%, control was22.2%,25.0%;clerks sub-group of experiment was11.1%,17.6%, control was26.7%,28.8%;workers sub-group of experiment was4.3%,2.4%, control was17.6%,14.7%;other-jobs sub-group of experiment was9.4%,9.4%, control was20.0%,31.3%.6.With the follow-up period of12and24month, the recurrence rate of the experimentalgroup of married patients was6.6%and10.3%, while the control group was20.9%and24.1%; the unmarried patients of experiment was8.3%,8.1%, while control was29.2%,28.6%.7. There was no statistically difference comparing the recurrent rate offollow-up period of12and24months.Conclusion:1.The urinary stones is most multi-component, including calciumoxalate and calcium phosphate mixed stones; The single component is onlyin a minority, such as calcium oxalate stones and uric acid stones.2. The recurrence rate of the experimental group was lower than the controlgroup with the follow-up period of12and24month, suggest that special dietaryguidance is helpful to prevent the recurrence of urinary stones.3. Comparision of the recurrence rate with the follow-up period of12and24month of the different genders suggest that special dietary guidance is helpful to bothfemales and males.4. Comparision of the recurrence rate with the follow-up period of12and24month of the different ages suggest that special dietary guidance is more helpful topatients of40s who pay more attention to health, and are willing to follow the specialdietary guidance. 5. Comparision of the recurrence rate with the follow-up period of12and24month of the different education suggest that special dietary guidance is more helpfulto educated people;6. Comparision of the recurrence rate with the follow-up period of12and24month of the different education suggest that special dietary guidance is more helpfulto civil servants and clerks.7. The recurrence rate of the2groups with the follow-up period of12and24month is almost constant.
