Objective:To analyze the dysfunction of left ventricular(LV) in type2diabetes mellitus(T2DM) by using two-dimensional speckle tracking imaging(2DSTI), and toevaluate in the early stage of left ventricular (LV) systolic dysfunction. To study therelevance between LV function and the levels of serum C peptide in T2DM.Methods:General echocardiography were done in60T2DM patients and60normalsubjects. Two-dimensional images were recorded from the apical four-chamber view,two-chamber view, long-axis view and three levers of LV short-axis viewsrespectively of LV for all studied subjects. The longitudinal peak systolic strain,circumferential peak systolic strain and radial peak systolic strain of endocardium andepicardium were measured in all of these views using special strain software. Thefasting or postprandial2hours serum C peptide of all T2DM patients were checked.Results:1. There were no significant differences between T2DM and normal groupsabout the measurements from general echocardiography (P>0.05).2. Compared with normal group, endocardial longitudinal peak systolic strain ofgroup T2DM decreased significantly(P<0.05) in all segments of LV, which occurredonly in a small part of segments about epicardial longitudinal peak systolic strain (P<0.05).3. Compared with normal group, circumferential peak systolic strain of groupT2DM decreased significantly(P<0.05), especially in base and middle segmentsendocardial.4. Radial peak systolic strain of endocardium and epicardium between the twogroups were similar.5.There was significant positive correlation between LV global endocardiallongitudinal peak systolic strain and fasting or postprandial2hours serum C peptide(r=0.32,0.62,P<0.05). There was significant positive correlation betweenLV global epicardial longitudinal peak systolic strain and postprandial2hours serumC peptide(r=0.55, P<0.05). There was no significant correlation betweencircumferential peak systolic strain, radial peak systolic strain,EF or FS and fastingor postprandial2hours serum C peptide.Conclusion:2DSTI strain could detect the dysfunction of LV in T2DM, which was difficult tobe detected by general echocardiography. The LV longitudinal systolic dysfunction ofT2DM was closely related with the level of serum C peptide especially that ofpostprandial2hours.
