Objective: By means of studying the medical records and related discusses onpain syndrome from Xue Ji’s writings in Ming Dynasty, we can comprehensively andsystematically summarize his empirical rules and clinical practices of the diagnosisand treatment of pain syndrome.Methods: After carefully studying Xus’s writings, to collect and sort out themedical cases with pain as the main symptom, analyze these cases according to thebasic system of Traditional Chinese Medicine, summarize his empirical rules andclinical practices of the diagnosis and treatment of pain syndrome combined withXue’s related discusses and explore Xue’s unique insights with extensive collection ofrelevant information in the comparative method to some academic issues.Results:Overwork, impassioned fail, misdiagnosis and mis treatment are the mainpathogens. According to the severity, pain can be divided as unbearable pain, seriouspain and slight pain. The natures and syndromes of pain are not one-to-onerelationship.Purple venules indicate a heat syndrome. The superficial venulesextended to nail means not dangerous syndrome. The diagnostic value of observationof the superficial venules of babies’s index fingers is limited. Xue emphasizes onpulse-taking followed by “combination of observation and pulse-taking in order todiagnose thoroughly”. In palpation of the abdomen, there is no equivalence betweenno-touch and excess syndrome, touching and deficiency syndrome. He emphasizes onexternal treatment as well as internal treatment in surgical diseases. He chooses various prescriptions and multi-selection of different prescriptions at the same time,sets the medicine-taken time in the morning or at night, or changes prescriptions afterthe former used up. He is expert in warm-recuperating, but does not discard purgation.Conclusion: Xue adopts the warm-recuperating method for the patients gettingpains due to the deficiency based on his clinical practices which breaks the restrictedsituation “no reinforcing method for curing the pain” in Jin-Yuan Dynasties and startsthe fourth period of pain syndrome treatment in Traditional Chinese Medicine. Thetreatment of pain syndrome by Xue not only reflects the feature of the warmingrecuperating method, but also consists with the basic principle of treatment based onsyndrome differentiation in Traditional Chinese Medicine.
