OBJECTIVETo explore the possible etiopathogenesis, diagnosis and treatment formalignant giant cell tumor of bone.METHODSSix cases from January2001to December2012were chosen, who hadbeen clearly diagnosed as malignant giant cell tumor of bone and werefollowed up successfully. Their clinical data were analysed and relevantliterature were reviewed.RESULTSThe six cases were all secondary malignant giant cell tumor of bone.The average internal time from initial benign diagnosis to malignanttransformation of giant cell tumor of bone was52.7months.Histopathologically, three cases were presented with fibrosarcoma, two withosteosarcoma and one with malignant fibrous histiocytoma. The treatmentsincluded radical surgeries for three, margin excision for one, high intensity focus ultrasound therapy for one and chemotherapy with palliative treatmentfor another. Mean follow-up time was22.8months, two cases had noevidences of diseases, two were alive with tumors and two died.CONCLUSIONSMalignant giant cell tumor of bone accounts for approximately5percentages of the total giant cell tumor of bone cases. Treatments for giantcell tumor of bone, like radiotherapy and recurrent incomplete curettage,might induce malignant transformation. Chromosome instability,heteroploid appearance and centrosome aberration might play an importantrole in the malignant transformation. Giant cell tumor of bone appeared localrecurrence over3years after the initial treatment should be considered thepossibility of becoming malignant. Early detection and radical surgery mightimprove the prognosis.
