Object:To raise the awareness of the related factors of clinical diagnosis and recurrence of giant cell tumor of bone with secondary aneurysmal bone cyst. Methods and Materials:the recurrence rate, age, gender, course of disease, the location of the tumor, alkaline phosphatase, erythrocyte sedimentation rate,high-sensitivity C-reactive protein and the coronal area of tumor, the patholog-ical fracture and the operative type of 23 cases of giant cell tumor of bone with secondary aneurysmal cyst and 58 cases of simple giant cell tumor of bone which have surgical treatment during the period from April 2011 to April 2016were retrospectivively analyzed. To investigate the relapse-related factors and the specificity of X-ray,CT and MRI in the diagnosis of giant cell tumor of bone with secondary aneurysmal bone cyst. Results:The recurrence,high-sensitivity C-reactive protein and the cases of the coronal area of tumor which was less than 14.40cm~2 in the two groups were significantly different(P<0.05).There was no significant difference in age, gender, course of disease, the location of the tumor, the positive rate of alkaline phosphatase, the positive rate of erythrocyte sedimentation, the pathological fracture and the operative type between the two groups(P>0.05).According to Campanacci's photographical grading,9 cases were grade?,6 cases were grade?,8 cases were grade?,there were 0 case of grade?,2 cases of grade?and 5 cases of grade?having a relapse. The higher the grade is,the easier it is to replase. An important imaging sign called“fluid-fluid levels”were ob-served in 0 case of X-ray,2 cases of CT and 3 cases of MRI of the preoperative imaging examination of 23 cases of giant cell tumor of bone with secondary an-eurysmal bone cyst. There was significant difference in the rate of observation of fluid-fluid levels in the three kinds of imaging examination(P<0.01).Conclusion:Giant cell tumor of bone with secondary aneurysmal bone cyst is more likely to have a relapse. The higher Campanacci's grading is,the greater the possibility of relapse is.Because of the high recurrence rate of giant cell tu-mor of bone with secondary aneurysmal bone cyst, clinicians should pay atten-tion on the cases which had not a high level of high-sensitivity C-reactive pro-tein and the location of the tumor was rich in blood while the area of lesion was comparatively large and use X-ray combined with MRI or CT to identify whether giant cell tumor of bone has secondary aneurysmal bone cyst as far as possible.
