Aim: To study the clinical manifestations and the correlation factors ofchildren’s acute paraquat intoxication prognosis and search for reasonableand effective treatments.Methods: The study included68patients from March2005to June2012who were subjected to acute paraquat intoxication.According to theamount of poison taken and clinical symptoms which were divided into threegroups(m ild,severe and fulminant),by retrospective analysis of clinicalmanifestation, auxiliary examination and prognosis among the groups andfollowing up the survival of these patients,explore the risk factors whichmay affect the prognosis.Result:(1)66cases were poisoned by taking orally in68cases and theother2cases were poisoned by the skin absorption.Gastrointestinalsymptoms were the most common,but lung and kidney is the most sensitiveorgan which could be involved in this disease.(2)Results among68cases,6cases was lost to follow-up,20cases died in the other62cases(32.26%mortality):mild type(n=22),all patients survived;severe type(n=26),8 patients died(30.77%mortality);fulminant type(n=14),12patientsdied(85.71%mortality).(3)As the degree of the clinical classificationdeteriorated,the mortality was increasing,there was statistical significationbetween each other and so did the white blood cell count,neutrophi,bloodurea nitrogen and serum creatinine(P<0.05).But there are no significantdifferences in blood gas analysis and lung function between eachother(P>0.05).(4) By following up,the renal function could fullyrecovered,the pulmonary fibrosis might be reversed.(5) The logisticregression analysis indicated that children who had been diagnosed withacute kidney injury and multiple organ dysfunction syndrome showed theworse prognosis,while acute lung injury,vomiting immediately, gastrolavagewithin2hours and the blood purification within6hours had little to do withthe prognosis.Conclusion: Acute kidney injury is a very important factor for the shortterm prognosis of paraquat intoxication and pulmonary fibrosis determinethe Long-term outcomes,lung injury may be reversed in the course of thedisease,so we should take active treatments.
