Objective:Through the specific immune globulin in vitro testing(allergens in food specific IgG in serum,serum specific IgE and total IgE)and the combination of acidophil percentage determination,explore the relationgship between the immune index and noninvasive fungal nasal sinusitis(noninvasive fungal rhino sinusitis,NIFRS), for its pathogenesis, subtype classification criterion and perioperative treatment to provide theoretical support。Methods:Randomly selected cases source:on May 1,2013-1 day in March 2015 in Dalian medical university second affiliated hospital otolaryngology hospital surgical treatment of fungal nasal sinusitis(fungal rhino sinusitis,FRS)33 cases,by clinical examination and pathology returns are determined for noninvasive。According to the clinical detection of acidophilic cell count,and serum total IgE is beyond normal,sIgE,s IgG test results and postoperative pathologic clues whether acidophil infiltration will be divided into positive fungus ball(fungus ball,FB)sinusitis group of 16 cases,allergic fungal sinusitis(allergic fungal rhino sinusitis,AFRS)group of 17 cases。On two group of patients with serum specific IgE,IgG,total IgE,peripheral eosinophil count testing and comparative analysis,explore NIFRS pathogenesis,classification and correlation with allergic factors。Results:1、FB,and AFS two groups of patients with peripheral blood eosinophilia positive rate have significant difference(chi-square=4.661,P=0.031),serum total IgE positive rate have significant difference(chi-square=7.127,P=0.009);2、FB in patients with total IgE class mainly in 1(81.25%)and AFS patients mainly in level 2(41.18%),the difference was statistically significant(Z=-3.189,P=0.001);3、FB in patients with serum sIgE class mainly in 0(50%),mainly in the AFS patients level 2(47.06%),there was no statistically significant difference between the two(Z=-0.655,P=0.512);4、FB in patients with serum sIgG class mainly in 0(56.25%)and AFS main at level 1,3 patients(29.41%),there was no statistically significant difference between the two(Z=-1.377,P=0.169);5、postoperative pathologic tip in 17 cases of eosinophil infiltration,sIgE positive patients compared with the histopathological classification of sIgE negative patients have obvious difference(chi-square=9.909,P=0.003)。Conclusion:1、NIFRS pathogenesis is various,not simple to fungal infection causes,specific allergic constitution and mechanism of allergy in illness and disease NIFRS play an important role in the process of evolution;2、NIFRS in different specific allergic constitution in the process of the disease,the disease evolution process are different。Can be divided into:fungus ball sinusitis and AFS two subtypes;3、s IgE、s IgG、total IgE、acidophil and other related immune indexes in the process of sinusitis and nasal polyps occur both can be used as an auxiliary examination;4、Idiosyncrasy and AFS is closely related to the disease。Fungal sinusitis subtypes of parting provide theoretical basis for clinical treatment to determine。...
