Objective:â‘ To investigate the clinical value of susceptibility weighted imaging inthe detection of hemorrhagic transformation(HT) after acute ischemic infarction.â‘¡Bystudying the risk factors of hemorrhagic transformation of cerebral infarction,to providebasis of scheme selection for clinical treatment. Methods:210patients of cerebralinfarction were collected in the Second Affiliated Hospital of Shandong University ofTraditional Chinese Medicine and the clinical data of patients should be intact.The patientswere examined by CT or MR,in order to clarify the diagnosis of cerebral infraction andexclude cerebral hemorrhage. Review of MR after treatment for a period of time(adopting1.5TMR routine T1WI,T2WI,T2FLAIR, T1FLAIR and SWI sequences).According to thepresence or absence of HT,divided the patients into HT group and non HT group,finally,theclinical,imaging and biochemical data of patients with statisticalanalysis.Results:â‘ 44HTwere found in210patients with acute cerebral infarction(the rateof HT was21%),and84patients with CMBs(the rate of CMBs was40%).â‘¡Single factoranalysis of general data between HT group and non HT group:the results showed thathistory of diabetes, high blood sugar,atrial fibrillation, large area cerebral infarction, smallvessels or lacunar cerebral infarction,cerebral embolism,positive urine protein hadsignificant difference(p<0.05).Included the analysis of the above factors which hadsignificant difference in single factor into the Logistic regression equation.Results showthat the large area cerebral infarction,urine protein,cerebral embolism(atrialfibrillation),high blood sugar were independent factors associated with the occurrence ofHT.â‘¢The difference of the positive rate among HT group and the control group had nostatistical significance(p>0.05).HT mainly distributed in the cortex and subcorticalareas,while CMBs mainly distributed in the basal ganglia-thalamus and cortex-subcortical areas,the distribution of groups had a certain correlation.â‘£The grading of CMBs andthetype of HT had a certain correlation.Conclusion:â‘ The sequence of SWI is sensitive indetection of HT and CMBs,significantly higher than the conventional MRsequences.â‘¡The large area cerebral infarction,albuminuria, cardiogenic cerebralembolism(atrial fibrillation),high blood glucose levels are independent risk factors forHT,can be used as a predictor of HT occurrence.â‘¢CMBs is not independent risk factor forHT,but the distribution of the two groups has a certain correlation.â‘£The grading of HTand CMBs has certain correlation,but the correlation is not strong,the reliability of theconclusion still need further study.
