Aim: This study is conducted to test the expression of VEGF and IGF-1in theserum and vitreous of patient with proliferative diabetic retinopathy. We furtherdiscuss the relationship of VEGF and IGF-1in serum and vitreous whichprovide a new avenue for the prevention and early treatment of PDR.Methods:32patients were enrolled in our study, including6male and16female (aging39~75years old, mean60.33years old; duration3~30years,mean10.91years).5patients were vitreous hemorrhage without retinaldetachment,5patients were both vitreous hemorrhage and retinal detachment,12patients were retinal detachment.10patients without diabetes mellitus wereenrolled as control, including4male and6female.(aging43~62years old,mean55.30years old; duration0.40~1.50years, mean0.80years), including2patients with Simple retinal detachment,5patients with Retinal detachment andvitreous hemorrhage, and3patients with idiopathic macular hole. Weinvestigated the history, age, gender, duration and the control of blood glucoseof these patients. Specimen preparation: peripheral venous blood was obtainedin early morning on an empty stomach for testing FBG, HBA1Cand HDL-C.We tested the serum and vitreous expression of VEGF and IGF-1by using enzyme-linked adsorption reaction. All data was indicated as mean±standarddeviation (ˉx±s). Rank sum test was used for the difference between two groups,and Spearman correlation analysis was used for the correlation between twosamples. P<0.05indicated as significant difference.Results: There were no significant difference between the PDR and controlgroup in age and gender (P>0.05). However, there were significant differencebetween the PDR group and control group (9.92±1.44pg/ml vs5.18±0.35pg/ml)in FPG, HbA1c(P<0.05), but no statistical difference was observed in HDL-C-c.Compared with the control group (0±0pg/ml,94.43(51.09,170.32)pg/ml,respectively), the levels of VEGF in serum220.27(111.90,484.24)pg/ml andvitreous1121.40(447.55,3118.80)pg/ml were significantly higher in the PDRgroup(P<0.05). Compared with the control group, the level of IGF-1in vitreouswere significantly higher in the PDR group (P<0.05). However, there was nosignificant difference in the level of IGF-1in serum between the control95(54.53,146.80)ng/ml and PDR group104.75(78.10,157.92)ng/ml.Otherwise, positive connection was observed in the serum level and thevitreous level of VEGF in the vitreous (r=0.62, P<0.05), instead of the controlgroup.Conclusion: The severity of proliferative diabetic retinopathy is closely relatedwith the duration of this disease, and the longer it persist, the worse it would be.VEGF and IGF-1play a key role in the development of proliferative diabeticretinopathy. In patients with proliferative diabetic retinopathy, the serum level of VEGF is closely related with the angiopoiesis of vitreous, which is a goodmarker for the diabetic retinopathy and provided an avenue for the treatment ofprevention of early treatment of PDR. The IGF-1in vitreous may be involvedin the development of PDR.
