Aim:To evaluate the methods and results of congenital severe talipesequinovarus deformity management in elder childern by using a simpleframe construct of Ilizarov external fixator combined with soft tissue releaseand osteotomy.Materials and methods:56feet in46patients with severe club feetwere treated by Ilizarov external fixator during the period2005.2–2012.2.All patients underwent soft tissue distraction and midtarsal osteotomy.Patients aged among8to16,and the average patient age at the time of theoperation was12.4years.Thirty of them were males and sixteen females.Results:Patients were followed up for a period ranged from1to8yearswith an average of48.5months. Out of56feet,29(51.78%) were rated asexcellent,18(32.14%) as good,8(14.28%) as fair, and1(1.78%) as poor.Excellent and good results (83.92%) were considered satisfactory, while fairand poor results (16.08%) were considered unsatisfactory.Conclusion: In the treatment of congenital severe talipes equinovarusdeformity in elder childern,the Ilizarov external fixator combined with softtissue release and osteotomy is a significant way to gain maintenance of correction and good function,and it is worthwile to apply in clinic work.
