objective To investigate the therapeutic value of neoadjuvantchemotherapy (NACT) in Ⅱ B cervical cancer.Methods Stage Ⅱ B cervical cancer patients(97persons) with inThe First Affiliated Hospital of Chongqing Medical University wasperformed with1to3courses of Platinum drugs combined taxanes byintravenous chemotherapy before radical hysterectomy, to observe thechange of tumor size, parametrial remission stuation,side-effects ofchemotherapy,situation in operation and postoperative pathologicalresults.Result The overall response rate of cervical local lesion was77.3%and included a complete response(CR)rate with17.5%,the overall responserate of parametrial improvement was91.8%and included a completeresponse rate with14.4%.The complete surgical resection rate was94.8%with35cases of keeping ovaries. The postoperative pathological completeresponse(PCR) rate was16.5%,interstitial infiltration≥2/3rate was22.7%,interstitial infiltration<1/3(including microscopic invasion) rate was43.3%,interstitial infiltration≥1/3but <2/3rate was41.2%,the rate of positive clinical resection margin is2,1%,pelvic lymph node metastasis rate was13.4%, vascular cancer embolus rate is4.1%,involving cervical canal was5.2%. The comparison of low differentiated and moderately differentiatedsquamous cell carcinoma,the effective rate of cervical local lesion were61.3%and84.5%respectively(p=0.01), interstitial infiltration>2/3ratewere35.5%and15.5%respectively(p=0.01), pelvic lymph node metastasisrate were25.8%and8.6%respectively(p=0.01).The overall response rate of cervical remission, interstitial infiltration>2/3rate,pelvic lymph node metastasis rate,para-uterus invasion in themoderately differentiated squamous cell carcinoma were lower than in thelow differentiated squamous cell. The main side-efrects of chemotherapywere hair shedding and the gastrointestinal symptoms. para-uterus invasionwas42.3%,Conclusion NACT of TP theray has a good curative effect toCervical malignancy in ⅡB,including effectively reducing the tumor size,improving para-uterus invasion, improving the operation resectionrate.NACT can keep the ovarian function of young patients,the effect of thistreatment in the moderately differentiated squamous cell cancer was betterthan in the low differentiated squamous cell cancer.
