Objective: To evaluate the clinical , histopatholoical and electron microscopical response of the neoadjuvant chemotherapy(NACT) for the patients with cervical cancer and to investigate neoadjuvant chemotherapy how to produce a effect. Methods: NACT was given to twenty-four patients with â… _b—Ⅱ_b cervical cancer and received neoadjuvant chemotherapy based of Cisplatin. Two weeks later, carry out radical surgery. After this, observed and analyzed the response in clinic, histopathology and electron microscopy both pre-chemotherapy core biopsies and post-chemotherapy surgical specimens. Results: After received neoadjuvant chemotherapy, the total respones rate (PR) was 66.7% (16/24) , the histopatholoical response was considered as grade â…¡ or â…¢ in 13 cases after NACT ,the rate of histopatholoical response was 54.2 %. The response of electron microscopyical ultrastructure was the distance broaden between cancer cell, the structure of desmosome grow no good, mitochondria and endoplasmic reticulum to swell, the groundsubstance in nucleus turned light to show caryolysis, part of them appeared cell apoptosis. The expression of p53 step up. Conclusions: NACT zoom out the tumor's bulk before operation, raise the opportunity to cut the tumor. It make tissue degeneration and necrosis, the cancer cell appear caryolysis and apoptosis, to start the anti-oncogene p53 to kill the cancer cell. Leading cell apoptosis is mechanism that neoadjuvantchemotherapy produce effect. NACT is a effectual treatment for the operation of cervical cancer.
