Objective: The aim of this research was to explore suitablescreening and diagnosis technology for early detection Congenital HeartDisease(CHD),to establish initially classification of "the villages andtowns–districts and counties-provinces" of screening,diagnosis andevaluation system in rural children CHD,to analysis of prevalence ofCHD in the rural area of Chongqing province.Method: Community considerd as the basic sampling unit inFuLing district of Chongqing province,and used integral sampling method,sampled10streets, villages and towns,including Longqiao,Lidu,Baitao,Jiangdong,Jiangbei,Chongyi,Lizhi,Dunren,Qingxi town,townshipNantuo as screening unit, brougt all0~3years children who were selectedsince community resident population into the screening object, and thetrained district and township pediatricians was screening objects accordingto7screening indicators,including "family history of CHD, dyspnea,cyanosis, special face, heart murmur, other congenital malformation,SpO2<95%", filled out screening form, children who had more than1item1and item screening indicators was suspected as "CHD children", andgranted the referral list,echocardiography was considerd as the diagnosis of core technology,children were diagnosed, evaluated and followed-up bythe pediatric Cardiologist.Finally,the screening results was analysed by thepediatric Cardiologist.Results:1.10005cases of children aged0~3years old wescreened had178cases with more than1item1and item positive screeningindicators,166of them underwent echocardiography,and60cases of CHDwas diagnosed,the prevalence of CHD was6‰.The detection rate ofCHD in special features and SpO2<95%was higher,85.71%、58.33%respectively,followed by other congenital malformations and the heartmurmur,respectively,55.56%、47.41%.2. After multiple factors regressionanalysis,heart murmur,special face had statistical significance (P <0.05),and its OR values was were5.010(1.748~14.359)、10.371(1.114~96.535)respectively.3. Simple CHD accounted for76.65%(46/60) in varioustypes of CHD, ventricular septal defect (simple and compound defect)accounted for50%(30/60), atrial septum defect (simple and complexdefect) accounted for41.67%(25/60),top the list.Conclusion:1.Classification of "the villages and towns–districtsand counties-provinces" of screening,diagnosis and evaluation systemestablished initially in rural children CHD.2.The prevalence of CHD wasabout6‰in the rural area of Chongqing province.3. Seven indicatorsshould combine application in screening,heart murmur,special face for theearly detection of CHD had significant meaning.4.To take health physical examination and screening of CHD, choose echocardiography correctly,evaluated by the pediatric Cardiologist is effective method in detection ofCHD early,and intervention timely,reduction the complications andnatural mortality of CHD in infants.
