Objective: Detecting the serum concentration of soluble intercellularadhesion molecule-1and procalcitonin measurement value,and correlationanalysis with Ranson score and APACHEⅡ score,in order to understandits ability to early predict the severity of the acute pancreatitis.Methods: Select15healthy volunteers and45acute pancreatitis patients forthe study,15healthy volunteers are the control group,45cases of acutepancreatitis patients divided into three groups, of which15cases diagnosedwith mild acute pancreatitis are the MAP group,15cases with severe acutepancreatitis combined infection are the SAP-1group, the order15cases ofsevere acute pancreatitis patients without infection are the SAP-2group. Allthe acute pancreatitis patients determine the sICAM-1and PCT values at thefirst day they admitted to the hospital. At the same time,all the acutepancreatitis patients accept Ranson score and APACHEⅡscore. The controlgroup determine the sICAM-1and PCT values at any day in the morning. Allmeasurements compared using analysis of variance between four groups; Thepatients measured value are correlation analysis with the Ranson score andAPACHEⅡ score.Results: The sICAM-1measured value of the control group is23.9±35.2ng/ml and the PCT measured value is0.20±0.19ng/ml, The sICAM-1measured value of the MAP group is441.88±50.83ng/ml and the PCTmeasured value is0.54±0.20ng/ml, The sICAM-1measured value of theSAP-1group is1081.3±71.4ng/ml and the PCT measured value is8.59± 3.99ng/ml, The sICAM-1measured value of the SAP-2group is1024.1±67.3ng/ml and the PCT measured value is0.93±0.29ng/ml. Comparing themeasured value of these four groups, The sICAM-1values of all the APpatients are higher than the sICAM-1value of the control group,P<0.05,ThesICAM-1values of the SAP-1group and SAP-2group are higher than theMAP group, P<0.05,But the comparation of sICAM-1values betweenSAP-1group and SAP-2group has no significant statistical difference,P>0.05. The PCT values of the MAP group and the SAP-2group are higher thanPCT value of the control group, P<0.05, The PCT value of the SAP-1group is obviously higher than the MAP group and the control group,P<0.01,The PCT value of the SAP-1group is higher than the PCT value of theSAP-2group, P<0.05. The sICAM-1and PCT values of the MAP group,SAP-1group and the SAP-2group are respectively correlation with theRanson score and APACHEⅡ score.Conclusion:Combined detection of sICAM-1and PCT can effectivelypredict the severity of acute pancreatitis,its predicted effect is positivelycorrelated with Ranson score and APACHE Ⅱ score. It can be use to earlyprognosis of acute pancreatitis and make appropropriate treatment measures.
