Objective: To investigate the soluble intercellular adhesion molecule-1 (sICAM-1), P-selectin (P-s), E-selectin (E-s) levels in patients with diabetic nephropathy, explore the early diagnosis of diabetic nephropathy clinical significance and the relationship between the sICAM-1, E-s, P-s and urinary albumin excretion.Methods:20 healthy persons in our hospital were randomly selected as normal control group, including 7 men and 13 women, aged 52±8.2 years. Their clinical and laboratory tests were normal. There is no diabetes, cardiovascular, liver and other serious disease.60 patients of the department of Nephrology, Affiliated Hospital of Yanbian University with T2DM were selected as T2DM group, including 28 male patients and 32 female patients, aged 52.6±2.8 years. Collect the general information of the patients and detect their HbAlC, TC, TG, HDL, LDL, Serum creatinine (Scr), creatinine clearance (Ccr) and urinary albumin. Evaluate the concentration of sICAM-1, P-s, E-s by Enzyme-linked Immunosorbent Assay (ELISA) and estimate Ccr and urinary albumin excretion rate (UAER) after collecting 24-hour urine. The T2DM group were divided into three groups by UAER:normal albuminuria group (UAER<20ug/min), microalbuminuria group (UAER20~200ug/min) and proteinuria group (UAER>200ug/min). Analysis of variance and linear correlation was used for statistical at last.Results:The concentrations of sICAM-1, E-s, P-s had a significant increase in the early stage of renal injury, and they also gradually increased with urinary albumin excretion in T2DM group and those were statistically significant compare with the control group (P<0.01).2. sICAM-1, P-s and E-s concentrations were positively correlated with HbAlC, TC, TG, LDL and Scr (P<0.01) and they were negatively correlated with HDL and Ccr (P<0.01).Conclusion: 1.The concentrations of sICAM-1, P-s, E-s increase early than the exception of urinary albumin excretion and closely related to the urinary albumin excretion rate in patients with T2DM.2. The concentrations of sICAM-1, P-s, E-s closely related to the glucose and lipid metabolic disorders in patients with T2DM.3. The concentrations of sICAM-1, P-s, E-s can predict the early indicators of renal injury.
