Objective: This study aim to explore the dynamic significance of serum PCT level as wellas correlation analysis in the traditional chinese medicine (TCM) syndrome type in sepsispatients.Methods: This is prospective cohort study in which41patients with sepsis,20patientswith severe sepsis and17patients with septic shock were included. Among those patients36patients were in qifen stage,15patients in yingfen stage and27patients in xuefen stage. Wetested the level of white blood cell count (WBC), neutrophil count (N), C-reactive protein(CRP),blood glucose on admission as well as the serum level of procalcitonin (PCT) on the1st,3rd,5th,7th day after admission in all the subjects studied. In addition, APACHEⅡscorewas evaluated. Analysis of variance of repeated measurement data was used to analyse therelevance of PCT level over time in TCM syndrome. Multi-classification logistic regressionanalysis model was used to explore the risk factors related to the severity of TCM syndromein sepsis. Drawing single factor and multi-factor ROC curve was to evaluate the diagnosticefficiency of PCT. Related statistical processing was performed by social survey statisticalsoftware package (SPSS)17.0versions. Test level (а) is0.05.Results: Compared with the sepsis group, the serum level of PCT in septic shock groupwas significantly elevated (P<0.05) on admission. Compared with the qifen stage group, theserum level of PCT in xuefen stage group was also elevated on the1st,3rd,7th day. The upshotof classification variables correlation analysis indicated that there was a strong correlationbetween TCM syndrome type and severity of sepsis (P<0.05).The variation trend of serumPCT level in three TCM syndrome was different, the falling range of serum PCT in qifenstage was higher than that of yingfen and xuefen stages. Pulse, APACHEⅡscore and PCTlevel on1st day were associated with severity of sepsis by the single factor analysis ofvariance (P<0.05). Multi-factor Logistic regression model analysis showed that risk factorsassociated with sepsis severity in TCM syndrome types were APACHEⅡ score and PCT levelon1st day (P=0.000and0.001,respectively). The APACHEⅡ score in Qifen stage was0.752times that of the xuefen stage (P<0.05), PCT level on1st day in qifen stage was0.906timesthat of xuefen stage (P<0.05). However, The APACHEⅡ score in yingfen stage was0.813 times that of xuefen stage (P<0.05), PCT level on1st day in yingfen stage was0.993timesthat of xuefen stage (P>0.05).The area of ROC of PCT on1st day with the highest diagnosticefficiency of sepsis was0.919,95%CI (0.852~0.987,P<0.05). However, the multi-factorregression model for predicting sepsis had high diagnostic efficiency as well (P=0.001).Conclusion: Elevation of PCT level is correlated to the performance of Ying fen and Xuefen syndrome in sepsis.The combination of PCT quantification with differentiation of TCM iscontribute to the diagnosis of sepsis and judgement of prognosis.
