ObjectiveTo study the incidence and influencing factors of the Macrovascular Complications in patients with type2diabetes mellitus and NAFLD.Methods1.179cases of type2diabetic patients were divided into two groups:combined with NAFLD group (103cases), control group (76cases).2. All the cases were draw fasting blood to test HbAlc, TC, TG, LDL-c, HDL-c, ALT, AST, ALP, y-GT, BUN, Cr, UA. Do the oral75g glucose tolerance test and insulin release test at the fasting time(without calories intook at least8hours) that test FINS, FCP,2H-INS,2H-CP; Color ultrasound detective liver, bilateral carotid and lower limbs arteries3. Measure the height (cm), weight (kg), waist circumference (cm), hip circumference (cm), then calculate the BMI, WHR, WSR,AIP,HOMA-IR,HOMR-p.4. Analyse the data with SPSS17.0software. Normal distribution measurement data were expressed as mean±standard deviation (x±s); Skewed distribution by logarithmically transformed mean±standard deviation (x±s); Pairwise comparisons using independent samples t-test; rate compared using the chi-square test; The influence factor model of the Macrovascular Complications in Type2Diabetes was explored by multivariate logistic regression. P<0.05indicates that the difference was statistically significantResults1.The two groups in terms of age, sex, duration was no significant difference (P>0.05). Fatty liver group WC, WHR, WSR, BMI was significantly higher than the control group, the difference was statistically significant (P<0.05)2.Two sets of biochemical parameters, HBA1C, BUN, SCR, UA, TC, LDL-C,HOMA-β, there was no significant difference (P>0.05); the combined fatty liver group ALT, AST, GGT, TG, FINS,2h-INS, FCP,2h-CP, HOMA-IR, AIP level significantly higher than the diabetic group, while HDL-C lower than the control group, the difference was statistically significant (P<0.05)3. With fatty liver group of major vascular complications were significantly higher than the control group, the difference was statistically significant (P<0.05). Lower extremity atherosclerosis incidence was significantly higher than the carotid atherosclerosis incidence of atherosclerotic peripheral vascular sclerosis, the difference was statistically significant (P<0.05)4. Logistic regression analysis showed that independent risk factors for type2diabetes with macrovascular disease are BMI, TG, NAFLD.Conclusion1. The NAFLD groups had higher insulin resistance, dyslipidemia and more obvious the incidence of obesity (BMI), especially central obesity (WC, WHR, WSR). Macrovascular disease in the NAFLD groups was significantly higher in control group.2. The independent risk factors of the Macrovascular Complications in Type2Diabetes was BMI, TG, NAFLD.
