Background:With the explosion of older people, the incidence of cardiac-cerebralthrombosis is increasing, and has become one of the most common causes of death.The Morbidity of cerebral infarction is at top of various diseases, and trends toward agradual incease. Thrombosis is the pathological process of multiple factors and steps.Revealing the pathogenesis of thrombosis, the clinical diagnosis and treatment ofthrombosis has an important guide sense. As a serious thrombotic diseases, Cerebralinfarction is characterized by suddenly and deadly and has greatly affected the qualityof life of the elderly. Prevention and treatment for cerebral infarction has extremelyimportant meaning. Semaphorin4D (Sema4D) is called CD100, belongs toSemaphorin family. Research shows that Sema4D and its receptor are expressed inmononuclear cells, B cells, endothelial cells and platelets, and Sema4D also isinvolved in the activation and the migration process of these cells. Sema4D and itsreceptor Plexin-B1, CD72play an important role in the process of the immune,nervous systems biology, as well as in the metastatic tumor growth process. Recentresearch suggests that Sema4D express on platelet surface, and play a big role in theprocess of platelet activation, atherosclerosis and thrombosis. It also found thatSema4D knockout mice showed thrombosis obstacles and atherosclerosis formreduce[1]. So that we think Sema4D levels rise is a new risk of the factors inthrombosis disease. And even though the research is already preliminary clarify the important significance of Sema4D in thrombosis, the research is limited to animalmodel at present. What role of Sema4D plays in the process of thrombosisdevelopment in the body? Which is the control mechanism of Sema4D? These arestill not very clear. For this reason, we discussed the correlation of Sema4D andthrombotic disease for the first time, to provide certain theoretical basis for theprediction, diagnosis and treatment of thrombotic disease in clinical.Objectives:1. To reveal the Significance of plasma sSema4D levels in cerebral infarction.2. To reveal the correlations of Sema4D gene expression and cerebral infarction.3. To explore the expression of Sema4D on surface of platelets in patients withcerebral infarction, and the connection with sSema4D.4. To investigate the expression of Sema4D on surface of platelets and in plasmain patients with cerebral infarction after treatment. And to discuss the relation of theexpression of Sema4D on surface of platelets and in plasma.5. To discuss the expression of Sema4D on surface of platelets in patients withcerebral infarction in acute stage and after treatment.Methods:The first part Testing the plasma sSema4D level in patients with cerebralinfarctionThe blood samples of cerebral infarction patients and healthy controls wereCollected.The plasma level of sSema4D was detected by using ELISA assay. Theexperiment was divided into3groups,acute cerebral infarction, cerebral infarctionafter treatment group, normal control group. In strict accordance with the sSema4DELISA kit instructions to complete the experiment process. Accomplished statistical analysis and explored the change of plasma sSema4D level in patients with cerebralinfarction.The second part Detecting the Sema4D gene level in patients with cerebralinfarctionThe experiment was divided into2groups,the acute stage of cerebral infarctiongroup and normal control group. Each blood sample collection was completed, thenextracted total RNA with lymphocyte isolation liquid immediately, and reversedsynthesis cDNA, and used Real time-PCR to test Sema4D gene mRNA expression oneach cell. Finally, analysised statistical data, and discussed Sema4D gene expressionof patients in acute stage with cerebral infarction.The third Part Dececting Sema4D level changes on platelet surface in patientswith cerebral infarctionThe experiment was divided into3groups, acute cerebral infarction, cerebralinfarction after treatment group, normal control group. Each blood samples aftercollected was immediately fluorescence staining, and detected Sema4D expression onplatelet surface by flow cytometry. Finally, analysised Sema4D expression on plateletsurface of patients with cerebral infarction.Results:The first part The change of plasma sSema4D level in patients with cerebralinfarction1. The plasma sSema4D level was significantly higher in patients with cerebralinfarction than that of control group (P <0.01).2. The plasma sSema4D level in cerebral infarction after treatment group andnormal control group had no significant difference (P>0.05). 3. The plasma sSema4D concentration in Cerebral infarction after treatmentgroup was significantly lower than in acute stage of cerebral infarction group (P <0.01).The second part The change of Sema4D gene expression in patients withcerebral infarctionSema4DmRNA expression in acute cerebral infarction patients was significantlyhigher than in the normal control group (P <0.01).The third part The Sema4D expression level on platelet surface in patientswith cerebral infarction1. The expression of Sema4D on platelet surface was significantly lower (P<0.01) in patients with cerebral infarction than in healthy controls.2. The expression of Sema4D on platelet were negatively correlated with thelevel of sSema4D in plasma sSema4D level (P=-0.328).3. The Sema4D levels was recovered in patients with cerebral infarction aftertreatment, near to normal levels, but no correlation with plasma sSema4Dconcentrations (P>0.05).Conclusions:1. The plasma Sema4D levels were higher in acute cerebral infarction group,than those of normal control subjects. Means that Sema4D may be involved in theprocess of cerebral infarction occurrence and development, and can be indicatedoccurrence of cerebral infarction.2. The gene expression of Sema4D was notably elevated in cute cerebralinfarction group compared with the control group. That means in the process ofcerebral infarction, the gene regulation of Sema4D may change. And Sema4F could be used as an indicator of cerebral infarction.3. Compared with control group, the expression of Sema4D on platelet wassignificantly decreased in cute cerebral infarction group. This phenomenon is relate tothe cerebral infarction.4. After the trentment, the plasma Sema4D levels and the expression of Sema4Don platelet returned to almost normal.It means that Sema4D is not only involved incerebral infarction, but also participate in the process of cerebral infarction.5. Sema4D may be a new risk factor for cerebral infarction. And detection ofSema4D is useful for diagnosis and clinical treatment.
