Discuss On The Diagnosis Related Groups And The Prospective Payment System Of Chinese Medicine

[Purpose] In the process of development of Chinese medicine service number isinsufficient, lack of effective incentive mechanism.This study attempts to introduce theconcept of DRGs to the Chinese medicine. Building a system of DRGs,which selects theadvantage of traditional Chinese medicine diseases, and in this system, Chinese andwestern medicine treatment according to pay with the same standard.In order to encouragemedical services provider active choice of Chinese medicine services, to increase theeconomic power of Chinese medicine services, to promote the development of Chinesemedicine to make a breakthrough.[Methods] Literature study: After a literature analysis, clear the plight of thedevelopment of Chinese medicine, proposed the idea to change this situation.Expertconsultation: in-depth interviews of experts in related fields, demonstration the feasibilityand the general idea of the DRGs of the Chinese medicine. Focus groupdiscussions:Convened the managers and clinicians in two Chinese medical institutions todiscuss, on the choice of the TCM disease and cost calculation method to collect views andsuggestions.Statistical Methods:Using the SPSS17.0to establish database,screening andproofreading the data,Using descriptive statistical methods to calculate the standard cost ofthe disease.[Results]1.The DRGs of Chinese medicine is feasibility,but because of the difference ofthinking of Chinese between western medicine,it must carry on the larger adjustment at thetechnical level, to adapt to the characteristic of Chinese medicine.2.Diseases selection principle: the advantages of Chinese medicine disease; Commondisease, frequently-occurring disease; Diagnosis is clear; Treatment method is stable andmature, disposal of the difference is small; Condition is simple, small variation, no or lesscomplications, complications of simple disease; Cure the standard clear. 3.It uses the historical cost method to calculate costs, based on the nearly three yearsaverage cost.In determining the standard cost, it is necessary to comply with the followingprinciples: cost calculation method should be based on actual needs; cost standards on thebasis of cost compensation, moderate earnings; costs grading standards in different regions,different levels of hospital.[Policy recommendations] This study is to stimulate the use of Chinese medicine,throught the DRGs of Chinese medicine.Combined with the development of Chinesemedicine status quo made the following recommendations: The same disease has the samecost standard in all hospitals;Achieve consultation in the departments between Chinese andWestern,by improving the cognitive of the Western staff on Chinese medicine;To develop areasonable single disease Chinese medicine services quality assessment system; improvethe construction of hospital information; Develop the cost adjustment mechanism;Establisha monitoring mechanism, improve the rewards and penalties; Expand publicity of theDRGs of Chinese medicine, and guide patients use Chinese medicine.
