For The First Time Total Hip Arthroplasty In The Treatment Of Refractory Hip Joint Disease Clinical Measures

Objective: Study was to analyze the surgical treatment of refractory hip disease cases thecountermeasures to explore the first total hip arthroplasty in the kinds of the disease.Methods:The reasons of biotype total hip replacement in63case (71hip) from March2009to March2012in our hospital with retrospective analysis, including thirty-eight hip bonyfusion （twelve cases got infection before operation， serious trauma of acetabulum inegitheen cases，einght got ankylosing spondilitis with hip ankylosis）nineteen got seriousbone defect of femur, six cases of hip dysplasia patients. Using Harris score function wasevaluated before and after hip surgeryResults: The group of62patients successfully completed the surgery, the incision healedall patients,2patients were lost to follow-up,1patient died of sudden cardiac death,followed up from9months to3years, with an average followed up of2.6years, theconventional X-ray inspection. Harris hip score was average92.6points after surgery. Thegroup, all patients leg length discrepancy at least poor1cm, up to10cm, average4cm,up topoor postoperative4.5cm, in most cases completely corrected, the postoperative meandifference of1cm after surgery. Post-operative dislocation of the prosthesis in1case,femur fracturing three cases, no wound infection, sciatic nerve injury after surgery.Aftersurgery the hip pain, activity returned to normal gait, the significant improvement in rangeof motion than before surgeryConclusion: Because normal anatomic structure great damage in refractory total hip replacement surgery, reconstruction of the hip joint normal physiological structure isdifficult, to remove the femoral head is difficult, to be fully prepared before surgery forprocessing may occur at any complications, preparing a variety of alternate surgical optionsto be careful at preoperative surgery operations, to detect and avoid all possible accidents.
