Abstracts: Purpose commenting on several different surgery methods, adaptation diseaseand clinical effects to find the best surgery method. Methods: To have a retrospectiveanalysis of the patients’ surgery situation, who are taken by our hospital for treatmentduring March1,2009to March30,2012. Four surgery method-improved capsular internaresection surgery, capsular externa complete stripping surgery, capsular sub-total resectionsurgery and liver resection. These four methods can be divided into four teams accordingto the different operation approach and whether to keep the capsular externa or not. Theclinical observation standard is time for surgery, blood loss, average time of staying inhospital, days with tubes, postoperative complications,(residual cavity infection, bileleakage, effusion), subphrenic infection, pleural effusion, after-surgery liver damage,death rate, local reccurrence, and conduct the statistical analysis. Results:①The surgerytime, the blood loss and after-surgery liver damage for not keeping the capsular externateam(team B) are all better than the team for keeping the capsular externa team(team A)(P<0.05);②The surgery time, the blood loss and after-surgery liver damage for capsularexterna complete stripping team(team B1) is not good as the Liver resection(team B3)(P<0.05);③The surgery time and the blood loss of the capsular sub-total resection surgery(team B2) is similar with team (B1), which means their differences have no statisticalmeaning. However, the average time in hospital and days with tubes and the after surgeryliver damage is not good as the team (B1).(P<0.05), the bile leakage rate is apparentlyhigher. Conclusion:①hydatid cyst external cystectomy is less traumatic and has loweroccurrence rate of residual cavity and lower local recurrence rate, which makes it the most necessary radical surgery for hepatic echinococcosis.②For some hepatic hydatid whichis near the aorta, important organs or unclear anatomical level. the capsular sub-totalresection surgery can annihilate the residual cavity effectively, at the meantime, it alsoreduces the difficulties and risks for the surgery.③The scope of the partial in-activatorneeds to be standardized, avoiding the sever complication due to the not standard use of inactivator.
