Objective: To explore the surgical methods and clinical outcomes of malunited anklefracture. Methods: From march2010to August2012,13malunited ankle fractures weretreated in our department.All patients with fibular shortening or rotational deformity,while3cases of varus or valgus deformity and2of a widen syndesmosis.All deformitieswere evaluated based on pre-operatively MRI.Then different reconstructive techniqueswere chosen according to the type and Recht level of malunion, a lengthening fibularosteotomy was chosen in patients with fibular shortening or rotational deformity; Openingor closing wedge osteotomy Was performed in patients with varus or valgus deformity;functional fusion of syndesmosis was performed in cases of widen syndesmosis. Results:Thirteen patients were followed up with an average period of21months (6~28months).There were no deep infection,internal fixation failure,fracture nonunion,deformity recurrence. The postoperative AOFAS ankle score was higher than that inpreoperative(P<0.05).1case with the superficial infection of screw path,All these threecases were cured by Actively symptomatic treatment. Conclusion: MRI are important tochoosing surgical methods. An early realignment reconstruction of the length and rotationof fibula and the congruity of ankle mortise could improve the ankle function and patientscan get good prognosis.
