Objective:To compare the clinical efficacy of gradeⅢ,Ⅳsupination-eversion fracture and dislocation of ankle joint with manipulative treatment and operative treatment,and to discuss the most idealistic immobilization methods.Methods:From September 2007 to December 2008,60 cases of gradeⅢ,Ⅳsupination-eversion fracture and dislocation of ankle joint were treated in our hospital, and who all tallied with the criterion that should be subsumed or eliminated.We divided them into two groups randomly with the random number table.Then one group of 30 cases were treated by manipulative treatment(Conservative Treatment Group),the other by operative treatment(Surgical Treatment Group).The common conditions,treatment information and postoperative recovery were evaluated and compared.Results:All the patients were all followed,and there is no significant difference(p>0.05) at the joint function,pain,times of fluoroscopy,X-ray film,complications and the rate of perfectness by Mazur after treatments.But at the hospitalization days,treatment costs, there is obviously significant difference between two groups(P<0.01).Conclusion:There is no significant difference in clinical efficacy between Conservative Treatment and Surgical Treatment to gradeⅢ,Ⅳsupination-eversion fracture and dislocation of ankle joint.Two treatment options for the functional recovery of ankle fractures have no significant effect,which all can reach a very good result.However, Conservative treatment that has high safety coefficient and low treatment costs,can achieve the goals of reducing more medical costs for patients and creating more economic benefits for society.
