The Research Of Township Hospitals Inpatients’ Health Services Utilization In The New Health Reform

Objective: Through investigating the inpatient lectronic information of the sampletownship hospitals, analysising the situation and trend of the inpatient health careservice utilization in township hospitals, to evaluate the changes in residents’ serviceutilization and the hospitalization to provide the health policy recommendations andsuggestion for the reform. Methods: The survey includes the inpatient electronic patientinformation database of the samples township hospitals in2007-2010, to extract thebasic information of the patients, such as diseases or symptoms, treatment,hospitalization costs and other information, and through the descriptive analysis, thetrend chi-square test, the multivariate regression analysis and other methods forstatistical analysis. Results: the inpatient health services utilization in townshiphospitals is as follows:(1) the count of the inpatient was going up with development ofthe New Rural Cooperative Medical Scheme, but after the new health reform, the countis declined.(2) In general, in township hospitals the female were more than than themale, the ratio of the group <15years was going up, and over65years was the highest;(3) Injury and poisoning, pregnancy and childbirth, circulatory system diseases,respiratory diseases, digestive diseases were located the top5in inpatient diagnosis. Theconstitute ratios were17.82%,16.07%,16.76%,12.81%,11.91%;(4) the diseases of themale were concentrated on the injury and poisoning, circulatory system diseases,respiratory system diseases and digestive system diseases, the diseases of the femalewere concentrated on pregnancy and childbirth, circulatory system diseases, respiratorydiseases, injury and poisoning, digestive diseases and female reproductive system andbreast disease;(5) the age group <15years were concentrated on respiratory diseases,the age group between15-34years were concentrated on pregnancy and childbirth, theage group between35-54years were concentrated on injury and poisoning, the age group between>55years were concentrated on the circulatory system disease.(6) thesupport, antibiotics and vitamins with a higher proportion of96.22%93.01%,86.09%;(7) after the New Rural Cooperative Medical Scheme (between2007-2009), the use ofsystemic antibiotics, vitamins, anti-viral and hormones were going up, after the essentialmedicines zero slip,(2010) the antibiotics, the anti-virus and the systemic hormonebegan decline, but the vitamins is continue up. Below the age of15, the use ofantibiotics, anti-viral and hormone is the highest proportion;(8) the combined use of theantibiotics, hormones and vitamins was serious;(9) the hospital costs continued to risebetween2007-2009, and declined in2010; the impact factors of hospital costs changeswere age, length of stay, surgery, and the hormones use, antibiotics use with conjunction,and hospitalization year. Conclusion:(1) the goverment should attached the importanceto the effective prevention and treatment for the grassroots areas injury, poisoning,chronic disease and other diseases;(2) focus on the special groups of children and theelderly group health services utilization;(3) contral the management about systemicantibiotics, systemic hormones, vitamins, support drugs and antibiotics combination, topromote the rational safety medication, and the control the unreasonable health costincreases;(4) strengthen the management of grass-roots hospitals and improveperformance management measures to promote the medical staffs’ enthusiasm.
