Efficacy And Complications Of TVT Procedure And TVT-O Procedure In Treatment Of Female Stress Urinary Incontinence

[Objective]To compare the effectivity and complications of Trans-obturatortape (TOT) and Tension-free Vaginal Tape obturator technique(TVT-O) for treatment of stress urinary incontinence(SUI).[Methods]Seventy-three female patients with SU1were randomly allocated for TOT (n=33) and TVT-O(n=40) treatment. SU1was diagnosed according to the symptoms, gynecological examination, stress test and urodynamics. The subjective cure rates were evaluated by ICI-Q-SF before surgeries and6,12months after surgeries. The mean operation time,intra-and post-operation complications,intra-operation blood loss, postoperation time with urinary catheter, hospitalization days, and one-year follow-up result were respectively recorded.[Results]Two groups of operation time was statistically difference, TVT-O groups of operation time is short. Bladder perforation occurred in one patient (3.0%) in the TOT group and in one patient (2.5%) in the TVT-O group (P)0.05).Urine retention occurred in two patients (6.0%) In the TOT group and two patients(5.0%) in the TVT-O group (P>0.05)。 hip pain occurred in3cases (9%) in the TOT group and in12cases (30.0%) in the TVT-O group, the difference between the two groups (P<0.05).The TOT group blood loss more than TVT-O group.There was no significant difference in Obturator hematoma,hospitalization days and postoperation time with urinary catheter between the two groups。None urethral injury and tape erosion occurred in the two groups. One-year’s follow-up indicated that the recovery rate was96.8%and94.4%for TOT and TVT-O groups respectively. Both groups had an improvement in ICI-Q-SF after the surgery.[Conclusion]1、TOT with TVT-O is an effective method of treatment for female SUI2、TOT operation method is more safe, recent complication is relatively less.3, the TVT-O is more secure than the TOT fixed, and is not easy to adjust, the TOT tape can have a few activities, if postoperative dysuria occurred, urethral expansion effect is more significant than the TVT-O early.4、the TVT-O is relatively small surgical trauma, less blood loss, treatment for patients with simple SUI advantage is obvious.5、The TOT group overall cost cheap.6、The TOT group set long-term complications may be higher, need further observation.
