Study On Applicability Of Nutritional Evaluation Methods For Primary Liver Cancer Patients

Objection:Firstly, the author tried understanding the extent and characteristics of malnutrition in patients with primary liver cancer by means of nutritional comprehensive evaluation. And then select the more appropriate program by analyzing and comparing the efficacy of three universal comprehensive nutritional assessment methods and their correlation with traditional single indexes aiming to provide a reference for clinical application.Methods:A total of80cases of liver cancer patients hospitalized in Tianjin Third entral Hospital from August1,2012to March31,2013. Information needed was collected48~72hours after admission with self-made questionnaire. The questionnaire includes two subscales:the general condition and nutritional assessment. Nutritional assessment subscales include:(1) Comprehensive assessment scale:NRS2002, SGA and MNA;(2) Anthropometric indexes:BMI, TSF and AMC;(3) Laboratory tests indicators:serum Hb, PA, Alb and TLC. Consistency test between results graded by three scales as well as correlation analysis of individual indicators and integrated assessment scale were executed. At last, we determined the appropriate screening program for patients with liver cancer.Results:(1) The malnourished rates determined by traditional single index were as follows:BMI (15.0%), Alb (20.0%), Hb (22.5%) TSF (27.5%), AMC (32.5%), PA (77.5%), TLC (92.5%);(2) NRS2002indicted33.75%nutrition risk incidence of the patients and SGA indicated the incidence of mild to moderate malnutrition of22.5%. The MNA method showed45.0%malnutrition risk incidence.(3) No significance was found of the consistent on results tested by any couples,(p<0.05).(4) Significance was found of single indexes between groups created by SGA except TLC. Significance was found of single indexes between groups created by NRS2002except Albs. There was difference on BMI, AMC and Hb between groups divided by MNA, while no difference was found on TSF, PA, Alb and TLC. Negative correlation was found between NRS2002score and BMI, TSF, AMC, PA, Hb. Positive correlation was found between MNA score and BMI, TSF, AMC, Hb whlie we found no correlation between MNA score and Alb, PA, TLC.Conclusion:The evaluation results of nutritional assessment from NRS2002, SGA and MNA of patients with primary liver cancer are inconsistent. NRS2002and SGA. showed better correlation with traditional individual indicators than MNA, so NRS2002and SGA for nutritional assessment of liver cancer patients compared with MNA is more reliable.
