objective:To compare the difference of the closed reduction of gypsum external fixation for the treatment ofSandersⅡ,Ⅲcalcaneal fractures and open reduction and internal fixation on therapeutic effects andcomplications.To provide theoretical basis for Selection of treatment methods for the treatment of SandersⅡ,Ⅲcalcaneal fractures.Method:Retrospective analysis some SandersⅡ,Ⅲcalcaneal fracture cases who had been treated by the abovetwo methods in the first Affiliated Hospital of Shihezi University of Medicine from January2006toJanuary2010.144cases were got into research by the uniform diagnostic criteria, inclusion criteria andexclusion criteria,including72SandersⅡcalcaneal fracture cases which contains gypsum external fixationclosed reduction (poking group) in38cases, open reduction and internal fixation (cutting compound group)in34cases,72SandersⅢcalcaneal fracture cases (poking group in36cases,cutting compound group in36cases).To make comparative analysis of age, sex, and treatment time in every group.Postoperative trackingfollow-up,evaluation of fracture healing by X-ray,postoperative evaluation of patients1year of footfunction by Maryland foot score system,statistics postoperative complications,hospital stay and hospitalcosts,organize data and statistical analysis.Result:1.There was no statistically significant difference between poking group and cutting compound groupof two types of calcaneal fractures on age, sex, and treatment time(p<0.05).2.SandersⅡcalcaneal fracture:There was no statistically significant difference between poking groupand cutting compound group on Maryland foot score,excellent rate,B hler angle,Gissane angle,height ofcalcaneus and their poor value,heel pain and philip impingement syndrome(p>0.05);There was significantdifference between two groups on wound dehiscence,hospital stay and hospital costs(p<0.05).3.SandersⅢcalcaneal fracture:There was significant difference between poking group and cuttingcompound group on Maryland foot score,excellent rate,B hler angle,Gissane angle,height of calcaneus andtheir poor value,heel pain and philip impingement syndrome(p<0.05);There was significant differencebetween two groups on wound dehiscence,traumatic arthritis,heel pain,philip impingementsyndrome,hospital stay and hospital costs(p<0.05).Conclusion:1.There was no significant difference in reduction effect and excellent rate between Open reductionand internal fixation and Closed reduction on the treatment of SandersⅡcalcaneal fracture,but the formerwas Superior in treating SandersⅢcalcaneal fracture.2.There was similar therapeutic effect in treating SandersⅡcalcaneal fracture between Closedreduction and Open reduction and internal fixation,and the former costed fewer and spent less time. 3.The author recommend the use of Closed reduction of gypsum external fixation in treating SandersⅡcalcaneal fracture, and Open reduction and internal fixation as the preferred method in treating SandersⅢcalcaneal fracture if wound complications are effectively avoided.
